Self-Discovery Survey

Complete the following survey and post your results (bar graph) on your weebly site.
The following survey looks at twelve different areas important for success both in and out of
school. In each area are five statements: for each, you must indicate how true it is about
you by using the following number code:
1 = Never;

2 = Seldom;

3 = Sometimes;

4 = often;

5 = always

Once you have completed the survey, add the numbers of each area and plot the results in
a bar graph. Your graph will be evaluated for completeness, accuracy and neatness and
assigned a mark out of ten.
