DIRECTIONS FOR SPEECH & LANGUAGE SERVICES


REFERRAL

All necessary forms are available on the ASD-E PORTAL. Go to Education


Support Services on the top bar; then to DOCUMENTS section on the left bar
to Speech & Language Services.

All referrals should be made via the school team.

If the school team agrees a referral is appropriate then the Speech &
Language Services Referral Form should be completed.
This requires clearly identifying the reasons for referral and a signature
from the referring party (this can be any TEAM member).
The form should then be sent home and signed by the parent/guardian.
The parent/guardian should also complete the Speech & Language
Services Parent Questionnaire
NOTE: Referrals for students in grades 4 through 12 must also include
the Speech & Language Services Grades 4-12 School Questionnaire
completed by a school team member.
When completed the referral form and the questionnaire(s) can be sent to
Speech & Language Services via mail or your school Speech-Language
Pathologist (mailbox at school).
When these forms are received completed and signed, the students name
will be placed on the Speech & Language Services waiting list.

Overview of Speech & Language Services:

Grades K 3: Formal Speech & Language Assessment and Service Delivery in the
following formats
o

Mild to Moderate Articulation Delay/Disorder = Monitoring service with option for block
of sessions as determined by the SLP. This indicates that the student is assessed and
coaching provided. The parents are provided with a program and directions to work on
speech sounds at home. The SLP follows up in 3-4 months to monitor progress. If
progress is poor, the SLP may offer a block (of 6-10 visits) to work on speech sounds
directly with the child.
Moderate to Severe Articulation Delay/Disorder = Direct intervention provided in 8-10
session block with option for second block as determined by the SLP. Blocks are visits
where the student works directly with the SLP in individual or small group sessions at
school. Homework is provided for family to continue work on goals at home.
Mild to Moderate Language Delay/Disorder = Monitoring OR Direct intervention
provided in 8-10 session block with option for second block as determined by the SLP.
Blocks are visits where the student works directly with the SLP in individual or small
group sessions at school. Homework is provided for family to continue work at home.
Moderate to Severe Language Delay/Disorder = Demonstration service with support
from Educational Assistants. The SLP consults with student and EA on a monthly basis
to update programming and provide recommendations for continued speech &
language progress.
At risk Kindergarten students = Informal brief screening in classroom for specific
students upon request from classroom teacher. Small group intervention for at-risk
students may be provided.

REFERRAL

Referral for consultation will be accepted. Informal consultation with the referring
party will be completed and then focused screening and/or assessment and/or
consultation and/or recommendations may be provided by the clinician as appropriate.
Clinicians will address functional need(s)/concerns(s) identified by family and/or school
in the referral.
Recommendations should be followed through by the school/parent teams working
with student(s). A follow-up consultation and/or support with recommendations will be
made only upon request.
