Mike Englehart

Is President of Advocate Physician

Partners (APP), headquartered in Downers Grove, Illinois. APP is a
care management and managed care contracting structured
collaboration between the Advocate HealthCare system and 4,900
aligned physicians, identified in 2014 as the largest accountable care
organization in the United States with over 610,000 attributed lives.
Mr. Englehart began his career with Advocate Health Care in 2001,
and has served as the Regional Vice President for Advocate Christ
Medical Center and Advocate Trinity Hospital and most recently, the
President of Advocate South Suburban Hospital. Prior to joining
Advocate, Mike was a healthcare management consultant with
PriceWaterhouse Coopers, working out of its Chicago office. He holds
a masters degree from Lake Forest Graduate School of Management
and a masters in Organizational Leadership from Lewis University.

Ashley Gillihan Is a member of Alston & Birds Employee and

Executive Compensation Group. He is also a secondary member of

the firms ERISA Litigation Group.

Mr. Gillihan represents various corporate plan sponsors, insurers, HSA

trustee/custodians, financial institutions and third party benefit plan
administrators. His legal practice focuses exclusively on assisting
clients with various health and welfare benefit plan compliance and
litigation issues.
Mr. Gillihan lectures frequently and has published various articles on
health and welfare benefit plan related topics. He is a faculty member
for the Institute of Applied Management and Law (IAML). He is a
member of the technical Advisory Council to the Employers Council
on Flexible Compensation. He is also the former author of the
Flexible Benefits Answer Book by Aspen Publishers.
Mr. Gillihan is a member of the state bars of Georgia, Tennessee, and
Alabama. He is a 1996 graduate of Samford University's Cumberland
School of Law.

Joan ORourke, RN, MSN, CPNP As the Vice President of

Specialty Program and Portfolio Management for CVS/caremark, Joan

ORourke leads a team responsible for ensuring optimum financial
performance of each of our specialty therapeutic categories. Ms.
ORourkes role includes working with internal partners to execute
business plans, making operational enhancements, and identifying
new products and services. She is also the specialty representative for
our Enterprise Adherence corporate initiative.

Ms. ORourke previously served as the Vice President of Specialty

Pharmacy Infusion Programs and Nursing Services where she was
responsible for the design, implementation, and management of the
Specialty Pharmacy Infusion programs and Nursing Services Specialty
Pharmacy Programs. Prior to that role, she served as a Clinical Services
Manager for CVS/caremarks Specialty Pharmacy department, where
her responsibilities included the development, management, and
implementation of the departments clinical programs.
Prior to joining CVS/caremark, Ms. O'Rourke served as a Pediatric
Nurse Practitioner in the Division of Endocrinology at Childrens
Memorial Hospital in Chicago, Illinois. Among her responsibilities in
that capacity were the management of patients with endocrine
disorders and the management of pharmaceutical-sponsored research
trials.
Ms. O'Rourke received her Bachelor of Science degree in Nursing from
Northern Illinois University in DeKalb, Illinois, and her Master of
Science degree in Nursing as well as her Pediatric Nurse Practitioner
Licensure from Rush University in Chicago, Illinois. Ms. ORourkes
Nurse Practitioner residency training focused on the areas of pediatric
primary care, endocrinology, and pulmonary medicine.

Dr. Jane Forester, Medical Director joined Allied, Inc. in

November, 2012 as Allieds Medical Director. She holds a Bachelors

degree from Cornell University, a Masters degree

of California, Berkeley and earned her Medical Degree from
Philadelphia College of Osteopathic Medicine. She completed her
residency in Family Practice at Lutheran General Hospital in Park
Ridge, IL. Dr. Forester practiced medicine in the private sector for
many years before joining Allied. She is licensed to practice medicine
in Illinois.

As Medical Director, Dr. Forester is dedicated to working with

members to make certain that they receive the most appropriate and
cost effective care. Dr. Forester works collaboratively with AlliedCares
Large Case Managers, Allieds Medical Review Team, and Allied
Members Treating Physicians to ensure that Allied is taking care of
our members and their families every day.

Patrick Gabrione, Chief Operating Officer and Senior

General Counsel joined Allied Benefit Systems, Inc. in 1998. Pat

graduated from the University of Notre Dame in 1986 and Santa Clara
University School of Law in 1989. Following law school, Pat was a
litigation attorney at a law firm in Chicago, Illinois.
As Allieds COO, Pat is responsible for the overall management of
Allieds administrative departments, including claim operations,
Allieds call center, eligibility services, stop-loss and Flex/COBRA
administration.
Pat is admitted to the Illinois Bar and the United States District Court,
Northern District of Illinois. He is a member of the Chicago and
American Bar Associations.

Brent Hanfling, General Counsel Brent oversees all legal

matters involving Allied. Brent also works closely with Allieds clients
regarding compliance issues involving group health plans and
cafeteria plans. In particular, Brent assists Allieds clients with
navigating the intricacies of the Affordable Care Act, as well as
consulting with respect to ERISA, COBRA, HIPAA and any other federal
law governing these plans.
Brent graduated from Northwestern University in 1991 and
Northwestern University School of Law in 1994, Magna Cum Laude.
Immediately following law school, Brent clerked for the Honorable
David Sentelle on the United States Court of Appeals for the District
of Columbia Circuit. Brent subsequently practiced complex
commercial litigation for several years in Chicago, Illinois before
transitioning into healthcare law. Brent is admitted to the Illinois Bar
and the United States District Court, Northern District of Illinois. He is
also a member of the Chicago Bar Association.

Gayle K. Nicholas, Executive Director of Human

Resources joined Allied in 2014 with over 30 years experience in the

human resources field. Most recently Gayle was Managing Director
of a Professional Employer Organization, Tandem HR leading all
operational aspects to including payroll, benefits administration, risk
management and HR consulting. Previously she was the senior HR
officer in various industries such as warehousing and distribution,
pharmaceutical manufacturing, real estate and public accounting.
As Executive Director of HR, Gayles primary focus is to attract and
retain the best talent for Allied while ensuring a culture that is
committed to taking care of our employees and their families.

Stuart Schapiro, Chief Information Officer has over 25

years of experience in healthcare and information systems. As

Allieds Chief Information Officer, Stuart is responsible for overseeing
Allieds technical operations. Stuart is responsible for ensuring that
Allied is compliant with applicable HIPAA technical and security
requirements.
Prior to Allied, Stuarts healthcare experience includes several years
of service at the American Medical Association (AMA). As Director of
Client/Server Computing, Stuart was responsible for designing and
implementing the AMAs client/server computing environment.
Stuarts formal education is in aerospace engineering, in which he
holds both Bachelor and Master of Science degrees from the
University of Illinois. He spent 3 years in the aerospace industry
leading various advanced commercial aircraft design efforts as well
as working on several research and development projects for the
U.S. Air Force and Navy. After leaving the aerospace industry in
1990, Stuart entered the healthcare industry where he has
continued to focus on computer and information related systems.

Rob Valerious

, Executive Vice President is a Shareholder and

Executive Vice President of Allied Benefit Systems, Inc.
headquartered in Chicago, Illinois. Allied is a national health
solutions company dedicated to helping employers take care of their
employees and their families every day. Rob oversees Allieds
Account Management, Marketing, and Underwriting Teams.
Instrumental in the design of many of Allied's products and services,
Rob has championed the development of Allieds Affordable Care
Plan, as well as other innovative medical and cost mitigation
strategies that have enabled Allied to become one of the largest
privately held administrators in the United States.
