Students are given medication calculation questions and are expected to pass with 100%. Students are also responsible for passing block meds in a simulation model in the lab appropriate at each level from freshmen to seniors. When a student miscalculates a medication, this is treated as a medication error incident. The student must complete a medication error reporting form. The medication error reporting form requires the student to calculate the safe and correct dose, investigate what the medication is commonly given for, what the adverse effects commonly experienced are, identify safety measures that may help to prevent similar medication errors from occurring and reflect on how the medication error reporting form has changed their view of medication calculations and medication administration to patients. Time Allotted 15 minutes to describe the learning activity and point it out in student packet. Each time a med error form is handed in it must be reviewed.
