This form should be used when the Seller identifies that the Property is subject to a waiver for the onsite
sewage system in the Agreement or the General Disclosure Addendum and should be signed at the time the
Agreement is negotiated and signed.
This document forms an integral part of the Purchase Agreement (the Agreement) dated the
