ONSITE SEWAGE (SEPTIC) ADDENDUM

This form should be used when the Seller identifies that the Property is subject to a waiver for the onsite
sewage system in the Agreement or the General Disclosure Addendum and should be signed at the time the
Agreement is negotiated and signed.
This document forms an integral part of the Purchase Agreement (the Agreement) dated the

for all that certain piece, parcel or lot of land described as follows to-wit:
("Property").

Pursuant to the Agreement, Seller has disclosed to Buyer that the Property is subject to a waiver for the existing onsite sewage
system.

TREATMENT / REPAIRS: The Buyer and Seller agree as follows (Mark appropriate box as to whether the Seller or
Buyer will be responsible for making any necessary treatment / repairs):

 Seller shall be responsible for any and all work necessary, not to exceed $

, to have the onsite sewage


system repaired, treated or otherwise corrected, including, without limitation and by way of illustration, additional
treatment or pressure dosing, such that an operating permit for the onsite sewage system may be reinstated by the proper
governmental authority. All such work necessary to cause the operating permit to be reinstated shall be completed and such
operating permit shall be reissued prior to the Settlement Date. Prior to Settlement, Buyer has the right to inspect any
work completed by Seller to assure such work necessary to cause the operating permit to be reinstated was
completed and that there is no malfunction of the septic system.
