YRPSLP and PRT Services Flowchart

List of children on PRT shared with YRPSLP Clinical Coordinator (which is to beshared with YRPSLP staff)
List of children in MTW shared with EIS Supervisors (which is to be shared with EIS staff)
Prior to enrollment in PRT/MTW, referring SLP/caseload EI to consult to
determine appropriateness of referral
Family participates in PRT/MTW
6 week consolidation period (minimum) follow both programs where no YRPSLP/PRT intervention
is provided (with exception of initial speech ax)
Caseload EI to connect with PRT SLP for all children following final PRT home visit to
discuss next steps in YRPSLP program. PRT EI to share report with YRPSLP
