Host, Date and Location of Tournament
The tournament is being hosted by Hardin Northern Youth Football on Saturday, June
18, 2016
All teams must be registered by Friday, June 3, 2016 to participate.

Teams and Tournament Play
The Hardin Northern Flag Football Fundraiser is open to players ages 8 and up.
Tournament format will be double elimination. All teams are guaranteed a minimum
of 2 scheduled games.
Games will be played 6 on 6 with an 8 player limit per team.
The cost is $40 per team This includes your team registration and lunch
deal.
Tournament Day Information
Team Check-in will take place between 8:30am 9:00am on Saturday, June 18,
2016.
Make sure your whole team is present when checking in. Please do not be late!
Games will
begin promptly at 9:40am. Game day schedules will be available upon arrival.
A lunch deal of 2 Hotdogs, 1 bag of chips and a bottle of water is available at the
school.
Hardin Northern will be handing out a great prize at the end of the day to the
winning team.

Team Registration
By Mail
Please pay by check if registering by mail
Step 1
Download the team/player registration. Fill out the registration form accurately,
neatly, and completely with a pen. Dont forget to include a Team Name.
Step 2
Include all contact information and signatures for all team members. A parent or
guardians
signature is required for any player under 18 years of age. Teams will not be
registered if
information or signatures are missing.
Step 3
Mail your whole teams completed Entry Form and total Entry Fee to:

Step 4
Once we receive your entry form in the mail and process it, we will send the captain
a
confirmation letter via email along with the official tournament rules.
Mail in entry forms must be received by Friday, June 3, 2016. If you have any
questions about the event, contact us at 419-306-5381. Please leave a voice mail if
your call is
not answered.
Player Changes
Registered teams may have a need to drop or add a player after they originally
signed up. In
order for the captain to make a legal change or addition, all player changes MUST
be made

prior to your teams first scheduled game. Exceptions will only be made for injured
players.

FLAG FOOTBALL WAIVER

THIS IS A RELEASE OF LIABILITY READ CAREFULLY
THIS FORM MUST BE READ AND SIGNED BEFORE ANY PARTICIPANT IS
PERMITTED TO TAKE PART IN ANY FLAG FOOTBALL EVENT.
EVERY PARTICIPANT MUST BE AGE 15 OR ABOVE AND MINORS (AGES 15-17)
MUST HAVE PARENTAL/GUARDIAN CONSENT.
In consideration of Hardin Northern Youth Football and Hardin Northern Scools
enabling me to participate in flag football games, I
agree as follows:
I fully understand and acknowledge that: (a) risks and dangers exist in my
participation in flag football activities; (b)
my participation in such activities and/or use of such equipment may result in my
injury or illness including but not
limited to bodily injury, disease strains, fractures, partial and/or total paralysis, eye
injury, blindness, heat
stroke, heart attack, death or other ailments that could causes serious disability; (c)
these risks and dangers may be
caused by the negligence of the owners, employees, officers or agents of HNYF and
HN Schools
the negligence of the participants, the negligence of others, accidents, breaches of
contract, the forces of nature or
other causes. These risks and dangers may arise from foreseeable or unforeseeable
causes; (d) by my participation in
these activities and/or use of equipment, I hereby assume all risks and dangers and
all the responsibility for any
losses and/or damages, whether caused in whole or in part by the negligence of
other conduct of the owners, agents,
officers, and employees of HNYF and HN Schools or by any other person.

I, on behalf of myself, my personal representatives and my heirs, hereby voluntarily

agree to release, waive,
discharge, hold harmless, defend and indemnify HNYF and HN Schools and its
owners, agents,
officers and employees from any and all claims, actions of losses for bodily injury,
property damage, wrongful
death, loss of services or otherwise which may arise out of my participation in flag
football activities. I specifically
understand that I am releasing, discharging and waiving any claims or actions that I
may have presently or in the
future for the negligent acts or other conduct by the owners, agents, officers, or
employees of HNYF and HN Schools.
I release all photos taken of me at Hardin Northern. I understand that they may be
used in
official marketing or for illustrations. We are not responsible for lost or stolen items.
