Team Registration
By Mail
Please pay by check if registering by mail
Step 1
Download the team/player registration. Fill out the registration form accurately,
neatly, and completely with a pen. Dont forget to include a Team Name.
Step 2
Include all contact information and signatures for all team members. A parent or
guardians
signature is required for any player under 18 years of age. Teams will not be
registered if
information or signatures are missing.
Step 3
Mail your whole teams completed Entry Form and total Entry Fee to:

Step 4
Once we receive your entry form in the mail and process it, we will send the captain
a
confirmation letter via email along with the official tournament rules.
Mail in entry forms must be received by Friday, June 3, 2016. If you have any
questions about the event, contact us at 419-306-5381. Please leave a voice mail if
your call is
not answered.
Player Changes
Registered teams may have a need to drop or add a player after they originally
signed up. In
order for the captain to make a legal change or addition, all player changes MUST
be made
prior to your teams first scheduled game. Exceptions will only be made for injured
players.
