INVITATION TO SUBMIT QUOTATION

The Philippine Institute for Development Studies (PIDS) invites all eligible supplier to submit quotation for the Re-installation of
Barcode program for the Library.

1. Specifications: (Job to done)

a) Re-installation of Barcode Program (V-Tech Barcode System)

2. Requirements: Submit a proposal with quoted price inclusive of VAT, indicating your total cost.

3. Terms and Conditions:

- Terms of Payment: 5 days after acceptance of the job.

4. To be registered and eligible with our pool of suppliers, interested parties must submit the following documents:
a) DTI/SEC registration
b) BIR registration
c) Municipal/Mayor/Business permit
d) Latest income tax return (ITR) with audited financial statements
e) G-EPS registration
f) Notarized statement that your company is not blacklisted or barred from bidding by any government agency, office or
corporation.
g) Notarized statement on the authenticity of the documents submitted.

Registration of qualified proponents is ongoing. Please secure copies of the affidavit forms at the BAC Secretariat c/o Mr.
Herminio F. Isip (893-9573), Room 404, NEDA sa Makati Building, 106 Amorsolo Street, Legaspi Village, Makati City, or
download the same at: http://www.pids.gov.ph.

5. All proposals duly signed by representatives of suppliers should be submitted on or before April 28, 2005 thru fax number 816-1091 and
addressed to the following:

Makati City

6. PIDS reserves the right to reject any or all the quotations submitted, if not in accordance with its specifications/requirement, and select
the quotation which it determines to be the most advantageous to PIDS.
