office and complete it
Use the folder to submit transcript requests
Submit requests for teacher recommendations

What is a college folder?

Each senior has a folder that keeps all forms together

This is how we keep track of what we have sent to


colleges on your behalf

Outside of the folder is used to request transcript for


EACH college/university

Inside are directions for the application process, a


release for parents, student self-assessment, and
parental questionnaire

What do I do with all this?


Read

Fill out the front with the information
needed for each college application

Make sure you have a valid email address that YOU will check
often

Invite teachers to write letter of recommendation, if needed

Download Secondary School Report(if needed) and turn in


with the folder or enter counselors email and form will be
sent by the school

Many of the secondary school forms are found in on


Freeman website under school counseling

How do I get a letter of


recommendation?
For

counselors: Fill out the self-assessment, the


parent/guardian form and the waiver

For

teachers:

Talk to them early so they have plenty of time to


prepare

What is the difference between


early decision and early action?

How do I get mid-year grades and


final grades sent?
At

the semester we will announce when you


should come request mid year grades

Make

sure you print and bring to us the


appropriate forms (mid-year and final)

At

end of year, final transcript card is completed


prior to graduation

I am planning to play sports in


college

Dont

forget to register with the NCAA clearing


house. A transcript of grades must be sent.

your on-line NCAA account
