Parents/carers expectations

Sarahs mother is supportive and is happy to support Sarahs learning, she would like her to attend the
homework centre and has brought her a diary to plan her work with her buddy

Health and safety information including a copy of a health plan if applicable

Sarahs intermittent hearing loss from Otitis media is an ongoing concern. Refer to AEO to ensure she
is undergoing her yearly Aboriginal health care check and review.

Skills, strengths and interests for learning

Sarah is a keen dancer and enjoys interacting with her buddy, she has recently taken up playing the
drums and is committed to her culture by performing in the community and school dance troupe

doesnt understand something, prompting her visually to answer question
and putting the lessons on EDMODO
Mrs Flanagan was identified from Sarahs PLP as a prospective Mentor
Syllabus outcome: EN3-1A: Student communicates effectively for a variety
of audiences and purposes using increasingly challenging topics, ideas,
issues and language forms and features

Measurable

Sarah will gain a confidence in class and can access EDMODO to interact
with her peers and clarify lesson content (teacher can monitor her access to
encourage participation)
Sarah has Mrs Flanagan for roll call who will monitor her hearing aid use and
engagement in her studies
Mark will be able to be signed off on his official workplace record once the
placement has been successfully undertaken
Syllabus outcome: EN3-2A: A student composes, edits and presents wellstructured and coherent texts

Sarah will be supported by SLSO and year 11 buddy and her work and
assignments scaffolded and sent to the homework centre (in collaboration
with classroom teachers)
Sarahs diary will be monitored by her buddy, Mother and classroom
teachers with a reward system in place for wearing her hearing aides

Sara will be taking on a leadership role in her dance troupes and
