SURVEY QUESTIONNAIRE

Dear Respondent:

This survey questionnaire is designed to determine the knowledge on


therapeutic methods of nursing students. Please indicate your response by
checking the box with the corresponding scales: 5-If you strongly agree; 4-Agree;
3-Fairly agree; 2-Disagree; or 1-if you strongly disagree.

1.) Know how to assess the patient’s physical and
mental health including the reaction to illness and
treatment regimen.
2.) Records and reports the results of nursing
assessment clearly and accurately.
3.) Can perform the nursing procedures efficiently under
the supervision of clinical instructor.

the clinical instructor.
5.) Know how to report and record the nursing care
rendered and the patient’s response to the care.
6.) Can maintain safe and effective nursing care,
whether rendered directly or indirectly.
7.) Have sufficient critical thinking ability for clinical
judgment.
8.) Able to identify cause-effect relationships in clinical
instructor.
9.) Knows how to collect and analyze data to aid in
problem solving.
10.) Can formulate effective nursing care plans for
patients.
11.) Able to establish rapport with patients and health
care team members.
12.) Knows how to communicate effectively either in
verbal or written forms.
13.) Have sufficient interpersonal abilities to interact with
clients, families, groups from variety of social, emotional,
cultural and intellectual background.
14.) Can explain nursing procedures in clear and
precise manner.
15.) Knows how to maintain and ensure safety of the
clients.
