Ensures patient and customer satisfaction meets or exceeds HCAHPS targets. Controls expenses and
meet or exceed productivity targets. Assure staff development through the use of evidence based practice.
2.

I feel that staff and patient satisfaction is my most important position responsibility. I strongly feel that
happy nurses make happy patients. When nurses have the support and tools that they need to do their
job effectively, they provide safer, quality care to their patients, thus increasing patient satisfaction.

3.

Have there been any special circumstances that have helped or hindered you in doing your position
this year? If yes, what were the circumstances and how did they affect your work?
I strongly believe that the change in leadership in the organization has greatly improved my performance.
My supervisor has supported me and I feel that we work well as a team. The acquisition by the new
company has certainly presented some challenges this year will multiple changes, but as a team we have
overcame these obstacles.

Accomplishments
1. List your most significant accomplishments or contributions during the past year. How do these
achievements align with the goals/objectives outlined in your last review?
One of my goals for 2015 was to reduce falls on each of my units by 25%. I felt that this was a lofty goal,
but for patient safety it had to be met! With further education of falls and the addition of bed/chair alarms,
we were able to reduce falls by over 50% this year- Huge Accomplishment!
2.

Since the last review conversation, have you performed any new tasks or additional duties outside
the scope of your regular responsibilities? If so, please specify.
In addition to my normal duties as a nurse manager, I also became Chair of the Fall Team. As mentioned
above, house wide we have shown a huge reduction in falls house wide.
Facilitated through my Internship, I also became actively involved in our nurse residency steering committee.
I am looking forward to conducting simulation experiences for our new associates in order to better prepare
them for practice here at Munroe Regional Medical Center.

3.

Describe professional development activities that have been helpful since last year (e.g., offsite
seminars/classes, onsite training, peer training, on-the-job experience, better exposure to
challenging projects).
Since last years evaluation, I have taken courses toward my Masters Degree at University of Central
Florida. I strongly feel these courses can help me to better educate the associates on my unit. These
courses include: Teaching Strategies, Curriculum Development, Evaluation of Nursing Education, Evidence
Based Practice, and Instructional Technology.

Goal Setting
1. What are your goals for the coming year and what actions will you take to accomplish these goals?
My first goal is to continue to work with the residency steering committee to plan and develop a successful
residency program. The residency program will begin in January and I plan to be a key educator in
delivering the education through simulation.
In addition to the residency program, I would like to continue to hardwire bedside shift report and hourly
rounding with my staff. I plan to accomplish this through being visible on the units during shift exchange to
ensure the process is being followed. I also plan to speak with patients about these initiatives when I am
doing my nurse leader rounds daily.

2. What could your supervisor/manager do to support you in doing your job and accomplishing these
goals?
To support me in reaching these goals, I will need leadership support during the allotted times so I can
devote time to the residency program. I will also need their support to assist me in observations/audits on
my unit as I have 64 patients on my unit each day.

3. What else would help you to do your job better and provide greater job satisfaction?
I feel that I could do my job more effectively if we could have free charge nurses on every shift. As we have
been staffing challenged, it makes it very difficult to manage and charge two units. I would love to see the
open charge nurse positions filled promptly. This will allow me to more effectively do my role as nurse
manager.
Development Planning
1. What kinds of professional development activities would you like to do during the coming year?
(For more information, go to http://hrweb.mit.edu/performance-development/goal-settingdevelopmental-planning/types-development-activities)
I would like to see the implementation of a house wide skill lab to ensure staff is adequately trained. In
addition, I plan to use more simulation activities to improve the quality of care on my units.
2.

What support or information do you need to complete these activities?

I will need the support and assistance from the house wide educators to facilitate the implementation of a
skills lab. This initiative will need the support of leadership to allow the educational hours for our associates.
