Description of artifact and how the artifact meets the standard:
For fall conferences in fourth grade the students complete a form to share with their parents at conferences.
The students share how they feel they are doing in each area, subjects in school they enjoy, areas where they
would like to improve, and a few fun things about themselves at that point in the school year. This form is in
the conference folder and we ask that students attend the conference with their parents. At the fall
conferences I do lead the majority of the conference with the students adding their input at appropriate times.

Reflection on artifact:
When it is time to complete the fall conference form I walk the students through each piece. I give ideas that
might be something for them to include. For example, I give ideas that multiplication facts might be an area
that needs improvement. We also discuss what concepts weve covered during to that point in the school year.
This is a refresher for students; I want them to have a wide range of ideas to include instead of just the current
topic in that content area.
The form we used at fall conferences this year was updated from previous years. We include a few places for
students to share their current interests and friends. This is beneficial to the students, parents, and me. It
allows students the freedom to share about themselves. I saw that parents learned new things about their
child and I also learned new things about the students in my classroom and what they enjoy doing in their free
time.
One thing we added to the conference form this year was a place for students to share what they needed for
school at that time. A large portion of my students wrote nothing or none here. Im not sure the students
understood exactly what this meant even after a class discussion. Also, at only six or seven weeks into school,
their materials are usually still in decent shape. Next year I dont think Ill include this component on the fall
form. It may fit better on the spring form as well past the half way point of the year and materials may have
run out, lost, or in poor condition. This would give students a chance to share with their parents their supply
