organization that is contracted by both medium and
large corporations to address the health needs of
Americas employees and implement work site
interventions

COMPANY INTRODUCTION SCRANTON IRON WORKS (SIW)

Scranton is located in Lackawanna

County, Pennsylvania
Scranton Iron Works was founded in

1887
SIW processes iron shipped from

regions north of the Erie Canal


Employees weld and fabricate steel to

make commercial high-rises and large


structural buildings

The company started with 35 employees and has expanded to 250
There are currently 220 males and 30 females employed

Employees between the ages of 18-55 years old


Demographic trend geared towards middle-aged population (30-40 years old)

175 of the employees have a GED or high school diploma, and 75 of them have a college

degree
Employees work between 40-60 hours a week making $25-$30 an hour
Yearly income between $48,000 and $86,000

STATISTICS
Lackawanna County is positioned in northeastern Pennsylvania and is ranked 56 out of 67

counties in the state by health outcome.


The amount of adults that smoke regularly is 23% in this county and 20% for the entire

state.
The amount of adults that drink excessively is 24% in this county and 17% for the entire

state.
On average, at least 50% of employees smoke at least once a day and 36% of employees

reported drinking alcohol 7 or more times a week.

SCRANTON IRON WORKS

Because of absenteeism, sick days, and reported alcohol related incidents on site,
the CEO of Scranton Iron Works contacted Horizon Health Solutions

A needs assessment was conducted in order to establish and prioritize the most vital
needs of the employees

There were several negative health behaviors found among residents of Lackawanna
and specifically SIW employees. Of the negative behaviors assessed, the most
pressing were smoking and excessive alcohol consumption

Cigarette smoking is the leading cause of

preventable death in the U.S.


The leading causes of death attributed to

smoking are: lung cancer, ischemic heart


disease, and chronic obstructive
pulmonary disease
$ 96.8 billion in productivity losses is due

to smoking in the U.S. annually

MISSION STATEMENT - SCRANTON IRON WORKS


The mission for Horizon Health Solutions is to significantly

improve the physical health status of all employees at


Scranton Iron Works.
Horizon Health Solutions will accomplish this mission by
implementation of a year-long program
Placing emphasis on:
Smoking cessation and alcohol abuse
Education, awareness, and support within the workplace

GOALS AND OBJECTIVES


Goals
To improve the health of the employees of Scranton Iron Works through a year-long health

intervention program
To decrease smoking prevalence among SIW employees
Smoking will be prohibited on SIW property
4 smoking cessation programs will be established
To decrease the prevalence of alcohol abuse among SIW employees
SIW will provide all employees with access to a substance abuse counselor

coincide with Healthy People 2020

PLANNING PROCESS HEALTH BELIEF MODEL
