Student Behavior Contract

Student
Student will begin this contract on _____ t o increase his
appropriate behaviors at school.

Daily Goals for Student.

1. I will behave in an appropriate way at school.
2. I will only use appropriate words and gestures at school.
3. I will stay focused on my work and let my friends work too.
4. I will accept responsibility for my actions.

How Goals will be monitored:

Teachers will document any inappropriate gestures, noises, or
comments. Each week a review of the data will occur. Mom,
student, teachers, and principal will be updated on a weekly
basis.
*Student reports that he has stopped this behavior. If this is the
case, we will stop monitoring after 3 weeks. If it is not the case,
we will monitor on a daily basis.
Rewards if goals are met:
1. Student would like to earn a good report home.
2. Student would like a good report to principal.
Consequences if goals are not met:
1. Begin daily monitoring.

2. Emails sent home to mom and principal on a daily

basis.
