Here are the staff meeting minutes from September 22, 2015.

Center Updates:
1. Scottsdale- Contact badging lady for city of Scottsdale if you do
not have SFAC badge. Windows7 update has been completed.
(If you need to take the course for SFAC security please attend
October14th at 10:00 at SFAC) Not mentioned in meeting**
2. Goodyear- There is a shredder under chair for all PIH information
and papers. Access in Goodyear, dont go in reception area and
not in playroom.
3. Glendale- Air card is hit or miss. Install of intranet Thursday Sept
22 is tentative date for complete functioning. Multiple emails
have attempted to address this issue. HonorHealth needs to do
the update. Paper chart instructions in an emergency. Glendale
open housewelcome to anyone to network and work with
others. Wednesday 3-6. New date of install is October 1st to begin
rebuild of equipment to supply Internet. ** Updated information
after meeting.
4. Mesa- Confusion with lockers. What they are for? Mesa lockers
are for all not needing to refrigerate. Where do we put stuff,
Mesa only fridge for Mesa Cases or Lockers for DV kits. **Chuck
Lines replied to question and okayed the use of the lockers for
DV Mesa kits** All others go to other fridge. Chain of custody will
read CAFV Refrigerator. **The F-EMR chain of custody now has a
dropdown of Mesa Lockers as an option.
5. Phoenix lab email sent by Jaime was helpful to all. **Remember exact times for collection must be written on the envelopes to
crime lab.
Cultures: Follow-up with lab with pre and post collection requirements.
Where to put it, who to identify you collected, who we are collecting
them on.
**CONFIRMED WITH LAB POST MEETING** Room temp prior to
collection is allowed. Chlamydia culture is allowed to be stored at temp
of degrees 2-25C prior to collection. Once it is collected, it must remain
as cold as possible and tested as soon as possible. Ideal is (28degreesC, frozen is suggested if long delay before testing).
All cultures collected must be picked up and taken to the lab by a staff
RN. Please leave the Gonorrhea (pink) swabs with the Lab Chain of
custody, on the pile of charts, on desk. This will be a reminder that the
Chlamydia (red) is in the refrigerator. Keep the Red in the locked
refrigerator (not mesa bins) following collection. The cultures must be
labeled Throat (back of throat across the palate arch like a strep swab
is collected). Endocervical (From the cervix) and Anal (anal folds).

Suggested questions for inquiring about previous sex is when was the
last time you has sex? if they say neverthen ask never? Like no
mouth, finger?, not only penetration. If the contact was all oral, anal
and vaginal then obtain all three and label them.
Shirley Righi open discussion and Q&A- introduced, annual report
added for ANCC, wanting to advertise the unique role of the forensic
nurse. Welcomes nurses to contact her to for capstones, magnet
conference, market the nurses.
-Doctorate programs. Graduate nurse education program with
HonorHealth. APN ASU, NAU and Grand Canyon University to granted
CMS. Community based hospital and this is the exceptional work.
-Coming to meeting is an opportunity for growth and support to our
program.
Todd:
Satisfaction surveys are complete. Numbers are great. Employee
scores, Area needing focus is having enough staff: problems need more
staff. We will run a class in December to help hire more nurses.
Internal Kudos: All satisfaction scores and reviews from community
partners is providing great feedback.
Court Prep is essential. Many nurses in court recently and nurses are all
receiving great feedback.
Good feedback is really encouraged. Be confident, proud and please
share it with Erin or Jill when it comes to you directly. Patients
feedback and documentation area all used to evaluate and show we
value you as an employee.
F-eMR is renewed for the next fiscal year and continues to be a key
necessity for our department.
Eileen Smith gave a large personal endowment donation will grow and
use the profit off of it.
Future growth: White paper for non-DV strangulation, response for
gunshot wound or trauma, officer involved cases getting possible
exams.
Death investigation- ME relationships.
History taking- medical relevance is essential. This is not an
investigative piece medical document. Suggestions of reason of history
that is needed to stay focused on medicalDo you think they are
adding more non medical safety issue? Non-medical injury.

Medical injury Documenting not examined, means you never looked

there. At minimum do a visual exam and if you truly do not look at all
then document not examined. Not every structure needs additional
assessment tools other than visual to state that you examined no
injury.
Diagnosis- question IPV/DV by history.
Both in diagnosis in the chart. What is the rational for on or the other.
Kim Day and JPW to define IPV/DV defines.
**It is suggested that if the patient has ever had a sexual relationship
with the offender this is now Intimate Partner Violence by history.
Domestic Violence by history is all other relations as a nursing
diagnosis. Several nurses are taking the Domestic Violence 24 credit
hour course online through IHS. Jackie has emailed a new registration
for the next course and it s encouraged for all.
Reminder with history of oral sexCollect swabs oral, circumoral, and
possible hands.
Fetal Doppler- use them and the protocol guidelines are at each center
and decline transfer.
Touch DNA- vary. Under clothing. Boob grabs. IAFN-boob grab swabs. **
We area likely leaning more toward doing the exam than declining in
these situations by they are a case by case basis. Please call Erin or Jill
before declining if there is question.
FeMR now has a dropdown for wait times, dispatch time. Please enter
them. This is for units of service tracking. Be honest and know we
understand that some delays are not because of the nurse. If there is a
long delay over the 1 hour time frame, please send an email to notify
the clock in and extended time needed for exam. Reminder that you
may need to look for or call for an eta and dont just wait for the officer
to find you, they may be waiting for you in another area.
Pager answerskeep pagers on you and answer in a timely manner.
Staff not needing to drive back to the Scottsdale center for the 30min.
Just finish up at the center you are at. You can leave. ** We are
watching travel and mileage closely as we are only allowed a certain
amount in our budget (recent months far over with the increased
number of exams). We must all do our part to make the trips to do the
hundreds of exams, a dual trip to deliver supplies if they are at the
office in Scottsdale. **

Prescriptions for patients must be given to those we have considered

as good candidates. Reminder to review patient allergies, and when
giving Flagyl, do pregnancy tests on all that have the ability to get
pregnant, including those stating they have not had sex.
Participate in Share and Care. Even if you dont want to give, fill it out
to support our department as compliance for participation at 100%.

and Care.
