Dear Coach,
As you know, DCH SportsMedicine offers complete pre-participation physical examinations for
area athletes in accordance with the Alabama High School Athletic Association and the Alabama
Independent School Association. This year, the examinations will be held January 16, 2016 for
spring sports in the Sports and Medicine Complex at the Northport Medical Center. Please
refer to the enclosed schedule for your schools time. Additionally, our fall sports preparticipation physicals are tentatively scheduled for April 23rd. Please encourage your
athletes to attend one of these 2 dates when your school is assigned. We will not offer an
additional make-up date for the summer. The fee for physicals is $15 or $10 and a piece of
usable medical equipment. (Refer to enclosed list) Usable medical equipment will be defined
as equipment in good condition with all of its working parts and pieces. If the equipment
comes as a set (for example crutches - pair) the entire set will need to be turned in for credit.
If you would like your athletes to attend, please see that they adhere to the following
guidelines:
1) There is a nominal fee of $15 or $10 and a piece of usable equipment (see above definition)
to be paid in cash by the athlete at the time of the physical. This money will benefit the DCH
SportsMedicine Fund, a part of the DCH Foundation, Inc. This fund provides service and
educational opportunities to area schools, recreational sports organizations, and
individuals. For more than 24 years, the DCH SportsMedicine Fund has provided protective and
preventive athletic equipment to area athletes and medical supplies and equipment for area
high school training rooms.
Athletes who do not have payment the day of physicals will not be given a physical
exam. Please encourage your athletes to bring cash only and exact change if possible.
2) The medical history section of the physical form must be filled out and signed by the athlete
AND his or her parent/guardian. Please fill out forms in black or blue ink. We cannot perform a
physical exam unless the form is properly completed and signed by the
parent/guardian. Please consider giving out the forms early so the athletes will have plenty of

time to properly fill out the medical history section and get it signed. Please make sure you use
the most recent physical form.

Usable equipment will be defined as equipment in good condition with all of its working parts
and pieces. If the equipment comes as a set, then it must be donated as a set. The following is
