PETITION
UNDERSIGNED PETITIONER, unto this Honorable Judge,
most respectfully states:
1. That he is a Filipino citizen, of legal age having been born on
June 15, 1992, single, with residence at 8412, Maharlika Avenue,
Phase 5, Marcelo Green Village, Muntinlupa City, well within the
courts jurisdiction. His personal number is 09175591992. Attached
herewith as Annex A is a certified true copy of petitioners birth
certificate;
2. That he has been admitted to the practice of law in the
Philippines on April 25, 2018 under Roll of Attorneys No. 1234567.
Attached herewith as Annex B is a certified true copy of his
Certificate of Admission to the Philippine Bar duly issued by the
Supreme Court of the Philippines;
3. That he is of good moral character and has not been
criminally charged, much more convicted, of any crime involving
moral turpitude and he has no pending administrative case before the
Supreme Courts Office of the Bar Confidant. A certified true copy of
his Certificate of Good Standing issued by the Bar Confidant is
attached as Annex C. In addition, two (2) executive officers of the
local chapter of the Integrated Bar of the Philippines (IBP) in
Muntinlupa have vouched for his good moral character as confirmed
by a certified true copy of a certification enclosed as Annex D;
4. That he has paid his IBP annual dues for 2018 as shown by
a certified true copy of the IBP receipt enclosed as Annex E. He is a
member of the IBP in good standing, a fact which the IBP confirmed
by the enclosed certified true copy of a certificate issued in his favor
marked as Annex F. He has also paid the required professional tax
for the year 2018 as shown by a certified true copy of Official Receipt

No. 1007327 dated May 2, 2018 which is hereto attached as Annex

G;
5.That he has all the qualifications and none of the
disqualifications for the issuance of a commission as Notary Public
and that he has no other commission as Notary Public in any other
jurisdiction;
6. That as further proof of his genuine desire to become a
notary public, he has attached as Annex H three (3) specimen of his
official signature;
7. Additionally, he has attached in a separate paper marked as
Annex I for the courts ready reference three (3) of his passport-size
color photographs bearing his signature at each of the bottom parts;
8. Finally, he has paid the required filing fee as shown by the
attached receipt marked as Annex J.
PRAYER
WHEREFORE, PETITIONER RESPECTFULLY PRAYS THAT
A COMMISSION AS NOTARY PUBLIC BE ISSUED.

sworn to in accordance with law, do hereby state that I am the
Petitioner in the above-entitled Petition and, in my capacity as such,
caused the preparation of the foregoing, the contents of which are all
true and correct based on my own knowledge and on authentic
records.
I further certify that I have not commenced any action or filed
any claim involving the same issues in any court, tribunal or quasijudicial agency and, to the best of my knowledge, no such other
action or claim is pending therein; nor is there any other pending
action or claim involving the same issues in any court, tribunal or
quasi-judicial agency; and if I should hereafter learn that the same or
similar action or claim has been filed or is pending, I shall report that
fact within five (5) days therefrom to this Honorable Court.
