Goals
1. I will ask for help from a PSW, nurse, my instructor, or one of my peers
whenever I do not find myself prepared or knowledgeable enough to
perform a specific task such as transferring or walking a resident to the
bathroom on my own all throughout clinical until April 4th, 2016.
2. I will promote safety of a clients environment if I observe any potential
risks or hazards that are not aiding in a clients well-being by March
16th.
Resources to achieve my goals
One: I will ask other healthcare professionals (PSW, nurse, or my
clinical instructor) and my peers as well for advice on my performance
in clinical.
o Review the preparations for lab and take notes on at least 2
concepts or terms that I do not understand such as that from the
Canadian Fundamentals of Nursing (Potter P. A., Perry A. G.,
Stockert P. A., & Hall A. M., 2014) or any of the Panapto videos.
o I will use my reflective journal to write down at least 2 tasks I am
comfortable and knowledgeable about and feel competent
enough to perform.
o I will also read the Professionalism (2007) article from the
Registered Nurses Association of Ontario (RNAO) to enhance my
understanding on how to refuse tasks and ask for help in a
professional manner whenever I do not feel competent enough to
perform a specific task.

Two: Informing my clinical instructor, other nurses, or the head of the


facility about the safety hazard my resident has encountered in order
to promote safety and minimize any harm done to the resident
o I will read and take notes from the Best Practice Guidelines
(BPG): Promoting safety alternative approaches to the use of
restraints (2012) from the RNAO website.
o I will also read the article on Restraints in the Practice Standard
(2009) from the CNO website in order to enhance my
understanding on restraints used in the long-term care setting.

Evaluation
One: I will know whether or not I am competent to perform a task from
discussing what I have learned from the Professionalism (2007) article,
the 2 concepts I do not understand, and the 2 tasks I have been able to
perform in a conference with my clinical instructor.
o This will allow me to feel more prepared to respond to a situation
where I do not feel comfortable appropriately and become more
knowledgeable about performing a task so that, in the future, I
will feel more competent to do that task.

Two: I will know I have attained proper knowledge on health and safety
risks once I am able to refer back to the Promoting safety alternative
approaches to the use of restraints (2012) and Restraints (2009)
articles whenever I have been questioned about my residents safety.
o This will allow the resident to feel comfortable in his/her
environment without any potential harm to their health and wellbeing. It will also help me to become more aware of their
surroundings, making it safer for them as much as possible.

1. I will identify 2 of each of the following: strengths, weaknesses, and the
objective and subjective health needs of one resident using proper
relational practice and assessment skills learned in lab by March 28th.
2. I will recognize 3 new beliefs and values of my resident and I through
therapeutic communication by March 21st.
Resources to achieve my goals
One: I will review the therapeutic communication information from my
NURS 1001 and NURS 1002 notes in association with geriatric patients
in order to enhance my relational practice skills.

Two: Reviewing my Ethics and issues in contemporary nursing


(Burkhardt M. A., Nathaniel A. K., & Walton N. A., 2014) textbook, NURS
1001 notes on therapeutic communication, my weekly reflections, and
my textbook, Ebersole and Hess Gerontological nursing and healthy
aging (Toughy T. A., Jett K. F., Boscart V., & McClearly L., 2012.).
o I will also read Embracing cultural diversity health care
developing cultural competence (2007) from the RNAO BPG.

Both: I will read the BPGs: Establishing therapeutic relationships (2002)


and Person and family centred care (2015).

Evaluation
One: I will know I have achieved this goal once I have written down 2 of
the residents strengths, weaknesses, objective, and subjective health
needs. This will enable me to use this information to provide care,
ensuring to be considerate, accurate, appropriate, efficient and safe.

Two: I will know I have identified 3 of my new beliefs and values as well
as that of the resident once I have written and dated my reflections on
them. This will allow me to understand diversity in culture, beliefs, and
values and me to develop mutual trust and a better relationship with
my resident.
