Ashley is a 22 year old female and is having trouble dealing with the guilt from her fathers

death, obsession with her weight and stress living with her mother. Ashley is an excellent student
and has a strong support system from a few close friends. This is Ashleys second meeting. The
confidentiality policy has already been discussed and agreed upon. We talked about several
issues. Some of the issues presented last week were her regret of not forming a closer bond with
her father before his death, the fear of gaining weight, and her unhealthy relationship with her
mother.
Throughout our session I used eye contact and played soft music in the background so
Ashley was relaxed and comfortable. Using a calm tone of voice, I introduced myself and asked
Ashley how she would like to be addressed. After asking what brought Ashley into my office, I
explained my purpose is to help alleviate her issues by working together. I made the commitment
to Ashley that I will uphold qualities such as honesty, reliability and respect. Our main priorities
are to work on the guilt Ashley feels from her father passing away without establishing a closer
bond to him, the constant struggle of living as a former anorexic and the frequent verbal fights
she has with her mother. We will establish feasible goals that target the problems warranting high
priority.
Goal: Ashley will decrease level of guilt
Objective: Ashley will document her favorite memories of her father.
Task: Ashley will watch at least one home video of her father by our next session
Task: Ashley will write a letter to her father and bring it to our next session
Objective: Ashley will attend grief counseling at least three times a month
Task: Ashley will purchase the book On Grief and Grieving
Task: Ashley will read one chapter a week from the book On Grief and
Grieving
Goal: Ashley will increase her knowledge of proper nutrition
Objective: Ashley will keep a food journal
Task: Ashley will write down her meals for a week including breakfast, lunch,
dinner, and any snacks throughout the day.
Task: Ashley will bring the journal to our next meeting on March 12, 2015 so we
can measure her calorie intake from the previous week
Objective: Ashley will create a positive notepad on her phone
Task: Ashley will find at least four inspirational quotes by our next meeting on
March 12, 2015 that are focused on body-image
Task: Ashley will add the four quotes to a notepad application on her iPhone and
read them whenever she feels anxiety about her weight

We, the undersigned, agree to the objective in the following plan:
a. We will have at least two meetings a month that work accordingly to Ashleys
schedule.
b. Ashley will document her favorite memories of her father and watch at least one
home video by March 12, 2015.
c. I will find grief counselors and Ashley will attend grief counseling at least three
times a month.
d. Ashley will create a food journal by our next meeting that documents her food
consumption throughout the day such as breakfast, lunch, dinner, and snacks.
e. Ashley will use her iPhone notepad application to add four quotes on positive
body-image and read them when she feels anxiety about her weight. Ashley will
bring the quotes to our next meeting.
