Demonstrated professional responsibility and
accountability to collaboratively plan, implement
and/or evaluate a health promotion project relevant to
the community population served, using elements of
the course website such as the learning system and
regular, punctual communication with preceptor and
faculty.

Sarah consistently demonstrated professional


responsibility by asking for clarification if in
doubt. She is a mature independent student
who demonstrated a strong commitment to
her placement and the projects put forth to
them. She kept current by utilizing the
blackboard frequently and appropriately.
Written assignments were excellent.
Communication was excellent. She was
flexible when just prior to midterm the ED at
the agency requested that they switch the
focus of their project.
Sarah demonstrated strong knowledge of the
health promotion education, sensitivity of
population in her assignments and
presentations. At the poster presentation, she
was clear and comfortable with her project
about assessing the issue of smoking among
staff at the YSS. Like her project partner,
Sarah was able to engage both the faculty and
students in her experiences at the YSS at the
presentation by answering their questions.
Her praxis notes were well done. Sarah
demonstrated good insight while dialoguing

with the youth and staff. In her LEARN, she
reflected on the importance of diversity of the
term homelessness and some of the struggles
surrounding obtaining some of the
determinants of health ie. food, health,
support.
Demonstrated ethical respectful and professional
practice when working with community organizations
and the population with whom they work.

Sarah was very genuine and professional in


all her interactions with me. She was very
confidential with her agency and population
issues. She was respectful in all
communication. She was able to develop a
poster with her project peer. She was flexible
and independent to gain the most experience
of the placement. She ensured that she was
prepared for clinical each week.

Demonstrated knowledge of relational practice in
project development, respectful of group process,
leadership/followership, and collaborative practice to
meet the needs and build capacity of the population in
the given context.

Demonstrated self-regulation by engaging in selfreflective practice and identifying learning needs


through self-assessment that align with the SONs
program goals and the Community Health Nursing
Standards of Practice in Canada.

Sarah and her project partner worked well


together. They were respectful and shared the
workload in relation to the praxis notes and
poster presentation. She developed a strong
relationship with both of her preceptors. Sarah
is very cheerful, positive and receptive to
constructive feedback.
Sarah managed her time well and requested
clarification when necessary. She was able to
meet the independent components of the
course as well as the team work. She met all
the course requirements as demonstrated by
her project, praxis notes, attendance (hours)
and evaluations. Her time management skills

were excellent.

Sarah's comfort level with the homelessness of youths was increased with this semester. She was a pleasure to work with and I wish her all the best as
she proceeds through the program. Again, her maturity and acceptance of change assisted her in having a successful semester.
