illness.
Students graduating from this program are prepared to work with people of all ages and genders (individuals, families, groups,
communities and populations) in a variety of settings.
Students graduating from this program are prepared to work with people of all ages and genders (individuals, families, groups,
communities and populations) in a variety of settings.
Graduates will learn to continuously use critical and scientific inquiry and other ways of knowing to develop and apply nursing
knowledge in their practice.
Students graduating from this program will be prepared to demonstrate leadership in professional nursing practice in diverse health care
contexts.
Graduates will be prepared to contribute to a culture of safety by demonstrating safety in their own practice, and by identifying, and
mitigating risk for patients and other health care providers.
Students will demonstrate the ability to establish and maintain therapeutic, caring and culturally safe relationships with clients and
health care team members based upon relational boundaries and respect.
Graduates of this program will be able to enact advocacy in their work based on the philosophy of social justice.
Graduates will effectively utilize communications and informational technologies to improve client outcomes.
Graduates will be prepared to provide nursing care that includes comprehensive, collaborative assessment, evidence-informed
interventions and outcome measures.
Year One Goals
Define and describe the term 'self-regulating' and what it means to a part of a 'self-regulating' profession. Build a sense of identity
between building a sense of self and profession.
Demonstrate the ability to work with the aging populations in the residential settings.
Recognize the meaning and relevance of the five foci within the nursing program.
Define the ways of knowing and learning with a focus on critical and scientific inquiry.
Recognize the experience of leadership in nursing and undertake a leadership role in peer groups.
Articulate their role as individuals and professionals in providing safe nursing care.
Establish and maintain a professional relationship with peers and an engaged, caring, and culturally safe relationship with older adults.
Understand the concepts of advocacy and social justice. Begin to develop self-advocacy skills.
Explain the relevance of information and technology skills that are essential to safe health care.

Explain the components of the nursing process. Perform a basic biopsychosocial assessment of an individual.
Identify evidence informed interventions and outcome measures with guidance.

Course Objective
Established therapeutic nurse-resident relationships
in residential long-term care settings.

Progress
Evidence/Indicators: (The student has ...)
- I have maintained confidentiality of any
information of a resident that any of my peers
have decided to share with myself for caring
purposes or tasks that need to be completed
- I visited the residents on spare time and started
up conversations of interest to the resident just
to keep them company and practice active
listening.
- I maintained boundaries and respected the
resident and their family whenever they preferred
to spend time alone and I always supported and
encouraged that they do so.
- I have facilitated a conversation with my
resident about a personally significant event
- I have used nurture, compassion, and
empathetic approaches toward the residents in
long-term care whenever they felt lonely, sad,
etc.

Performed skills relevant to situating an individual

within his/her personal, familial and community
context

- I completed my care cards on the different

chronic conditions to enhance my understanding
on prevention and management of them.
- I ensured that my residents caregiver was not
burdened with caring for the resident by asking if
there is anything I can do for them.

Objective

Did not
meet
objective

Developed and demonstrated skills in basic

assessment techniques relevant to the long-term care
population

- I have stayed up to date with news in the media

in relation to long term care and health care in
general (such as euthanasia)
- I have continuously observed my resident for
any signs or symptoms I noticed to be different
as part of an assessment.
- I have assessed for any changes the resident
wanted to have, living in the long-term care
home.
- I have engaged with a resident in decisionmaking that would benefit themselves and others
within the long-term care facility and then bring
up their concerns to someone in authority who
knows how to implement that decision.

Demonstrated skills in providing (resident) clientcentered support for activities of daily living

- I have maintained a balance between what is

required and what the resident prefers for their
physical and mental state ex. Using the commode
if they do not want to walk on their own
- I have always allowed the resident to do as
much as possible on their own without my help
until they asked for it ex. Letting them brush
their teeth while I hold the bowl for rinsing out
the mouth
- I remembered as many wishes and preferences
for extra care beyond the basic requirements,
such as lipstick or powder after pericare/dressing

Developed knowledge about the experience of

residents living in a long-term care setting

- I asked as many residents as possible their ideal

living situation (with their chronic diseases) vs
their actual living situation to understand their
perceptions of both and how I can modify their
actual living situation as much as possible for
them.
- I watched videos of interviews between nurses
and their residents to understand their point of
view of the process that brought them to longterm care.

Demonstrated safe and ethical clinical practice at the

level appropriate for a year one nursing student

- I have always made sure to check the room and

bathrooms of the residents for any physical
hazards to the persons physical or mental health.
- I made sure the resident is comfortable and if
not, I asked if I could do anything to help.
- I have never done a task that I felt I could not
handle alone by asking for help from someone
else who felt more competent. However, when I
did learn how to handle situations that I needed
help with, I made sure to try to handle them on
my own as safely as possible.

Participated in professional development based on

reflective practice and clinical inquiry

- I have completed every reflection required and

immediately reflect on how well I have
interacted with the resident and what I should
improve on.
- I associated many of the residents stories

(treatment in long term care vs home for

example) to what I have learned in my NURS
1002 class as case studies to better understand
how I could improve their well-being and what I
could do to reduce any harm.
Examined personal attitudes regarding the elderly
and other residents of long-term care homes

- I examined my values towards how one cares

for a resident adding the opportunity for
autonomy and empathetic approaches to my
values.
- The fewer stigmatizing thoughts/assumptions I
had towards the elderly, the easier it was for me
to work with them

Developed a basis knowledge of the clinical

manifestations and relevant nursing interventions of
chronic diseases

- Using care cards, I was able to understand and

apply any nursing interventions I researched to
the residents in long term care.
- I reviewed my NURS 1002 notes on clinical
interventions for common chronic diseases such
as delirium.

Identify 3 personal strengths developed in this placement.

1. I have become very efficient at planning, completing, and following up with any care I have provided
2. I have developed great interpersonal skills such as communication and emotional intelligence.
3. I have learned to ask for help when needed, but to be as independent as possible.
Identify 3 areas requiring further development.
1. I will work on my time management being prepared for adverse events
2. I will remember (continuously) to check the room of any client I have cared for in order to minimize any possible hazards the
client may experience (falls).
3. I will remember to introduce myself to any client I will care for.
