We, BIENVENIDO ARANCILLO and GINA VILLARTA, both of legal age,
Filipinos and residents of Sagay City, Negros Occidental, Philippines, after
having been duly sworn to in accordance with law, hereby depose and say that:
1. That we are the parents of BENJIE V. ARANCILLO;

We recently knew that his name as reflected in his School records

(Bato National High School) and Diploma appears as JOSE BENJIE V.
ARANCILLO while on his Certificate of Livebirth certified by NSO
(Secpa) records and Certificate of Baptism records his name appears
therein as BENJIE ARANCILLO y VILLARTA which is his true and
correct name. However both entries refer to one and the same person;

3. We execute this affidavit for the purpose of establishing that his true and
correct name is BENJIE ARANCILLO y VILLARTA and for whatever
legal purposes it may properly served.
4. That we attest to the truthfulness of the foregoing recording to the best of
our personal knowledge and belief.
