How to Sign Up for Medi-Cal


Medi-Cal Center Address: Enterprise Self Sufficiency Center, 8477 Enterprise Way, Oakland.
Hours: M-F 8:30-12pm and 1-5pm.
1. Medi-Cal benefits cover: A primary care physician, checkups and preventive care, immunizations, regular OB/GYN visits,
family planning services, 24-hour emergency and urgent care, hospital care, prescription drugs, laboratory tests and services,
maternity care and well-baby care, eye exam and eyeglasses, health education classes, home health services, interpreter
services, transportation to your health care provider, for special circumstances, specialty care.

b. To sign up for Medi-Cal in-person, go to Medi-Cal Center in Oakland (address above).
c. Or, call (510) 777-2300 to request a mail-in application. For help with application, call (510) 777-2300
d. To sign up for Medi-Cal with a certified enrollment counselor, contact Alameda County Health Care Services
Agency at (510) 532-1930
2. *Persons who qualify for Medi-Cal:
a. Must be a California resident, and any of the following:

income levels)
c. Blind, disabled, under 21, pregnant, or are in a skilled nursing or intermediate care home
d. On refugee status for a limited time, depending how long you have been in the United States
e. A parent or caretaker relative or a child under 21 if:
i.
The child's parent is deceased or doesn't live with the child, or
ii.
The child's parent is incapacitated, or
iii.
The child's parent is underemployed or unemployed
f. Have been screened for breast and/or cervical cancer
g. If you are enrolled in one of the following programs:
i.
CalFresh, SSI/SSP, CalWorks (AFDC), Refugee Assistance, Foster Care or Adoption Assistance Program
h. *The following groups can now receive Medi-Cal regardless of the family's property:
i.
Children age 1-5 with income at or below 133% of the federal poverty level, Children age 6-18 with
income at or below 100% of the federal poverty level, Pregnant women and children under 1 year of age
with income at or below 200% of the federal poverty level.

1. Services offered: STI testing and treatment, contraception + emergency contraception, cancer screening and prevention,
pregnancy testing and prenatal services, abortion services, UTI treatment.

size.
4. If a low-income California resident and do not have insurance that covers family planning, or if insured but need to
keep family planning services confidential, you are eligible for Family PACT.
a. Services that come free with Family PACT include: Various birth control methods, STI prevention, screening and
treatment.
b. If interested in family PACT, tell the people at the front desk at Planned Parenthood that you would like to sign up
for family PACT to cover the cost of your appointment, and they will hand you a very short form to fill out. All
information is self reported, including income.

2. Services include primary care medical services, urgent care, family planning services, prenatal care, pediatric health
care for children of young adults, counseling, crisis intervention.
3. No appointment needed.
4. They accept Medi-Cal and other forms of insurance.
5. If you are uninsured, they can register you for Medi-Cal, Family PACT or HealthPAC when you show up to the
clinic, before you are seen for services.

Berkeley Free Clinic
All services completely free, confidential STI testing to all genders and limited acute primary health care, 18+ only.

1. Every Monday, Wednesday, Thursday, Friday offers acute primary medical care and TB testing
a. The clinic is able to see sore throats, ear concerns, some coughs, male genitourinary concerns, skin conditions, TB
testing, asymptomatic (routine) STI testing, peer counseling, pregnancy testing.
b. Same night appointments only. Services provided from 7pm-9pm.

as homeless) and a medic will run through some health screening questions with you before giving you an
appointment.

Order of Malta Clinic of Northern California
Clinic for the uninsured
Address: 2121 Harrison Street #110, Oakland.
Hours: Mon/Wed 8:30AM-12:00PM, 1:00PM-4:00PM and Friday 8:30AM-12:00PM.

3. Services provided include lab work, prescriptions, diabetes testing and treatment, checking blood pressure, chronic
concerns, immunizations, out-source x-rays.
Youth UpRising
Membership-based youth health and wellness services

1. Services offered: provides mental health care for transitional age youth, offers individual/group/family therapy, psychoeducation, clinical consultation, medication referrals, in addition to physicals, immunizations, treatment for acute and chronic
illness, STI testing and treatment, pregnancy testing, counseling to support youth struggling with sex and sexuality,
preventative health screening for asthma, diabetes, and hypertension, nutrition advice, acupuncture, massage therapy, and
sports & physical fitness exercise activities including basketball, hip-hop classes, soccer, etc.

All services provided free of charge if a member, so must go through free intake process to receive services.

Highland Hospital
Primary trauma center and county hospital in Alameda county.

Highland Same-Day Clinic
For serious, but non-emergency illnesses or injuries.
Address: 4th floor, Highland Care Pavilion of Highland Hospital Campus (1411 E. 31st St, Oakland.)
Hours: 8-5pm Mon-Sat.

a. Care for illnesses and injuries that require immediate, but NOT emergency attention
b. Care for patients who need timely access to healthcare, but who do not have a primary care physician.

RotaCare Richmond Clinic
Serves both adults and children who have no health insurance
Address: Family Justice Center, 256 24th Street, Richmond.
Hours: Tue 4-7pm, 2nd and 4th Saturday of each month.
