I learned how to create the nursing diagnosis based on


the scenario given (using previous assessment skills
learned in HEAS) deciphered which actual and potential
health issues needed to be addressed. This skill is
meaningful because it will be used throughout my career
as a nurse to focus the care directly onto the issue or
state of health of the patient while remaining under my
scope of practice. It is also a very important skill as it
gives me practice using the NANDA terminology which
is a universal language for nurses.

I would rate myself Fair to Good at writing nursing
diagnosis. I can usually conclude the right NANDA
diagnosis to use, using the scenario, but occasionally get
stuck and need help. To achieve this rating I practiced
the nursing diagnosis in lab, where the instructors
demonstrated how to create the nursing diagnosis, and
on my own. I also attended previous lectures in other
classes to get the base knowledge of this skill. I have
also done the readings pertaining to this skill and have
done some research into developing my understanding
of the topic of nursing diagnosis.

APPLY THIS LEARNING IN
YOUR CURRENT AND
FUTURE NURSING
PRACTICE

Currently I am applying this skill in lab, and it will be a


necessary skill to pass my CIA and to complete this
semester. We use the nursing diagnosis in lab to
determine the needed assessments and skills that we will
have to complete so that the patient in the scenarios
receive the right care and treatment. If you do not get the
correct nursing diagnosis, you will complete the wrong
assessments and skills and will not help the patient and
in turn will not pass the CIA or the class. In the future, I
will be using the nursing diagnosis very frequently and as
I learn new skills my application of this skill will change
and improve. I believe that this skill will be used with
every patient that I have to gauge what will be necessary
for me to provide the best possible outcome for this
client. To improve this skill I could take courses on
assessments, there is currently a course available through
the CLPNA titled: advanced care planning goals of
care. This course provides education in helping Lpns
think about, talk about, and document wishes for health
care outcomes and provides useful building blocks to
conversations about better goal outcomes and outcome
planning.
