Teachers Name: Jessica Lemon
Administrator: Eileen Oliver-Eggert & Sue Smith
Subject/Grade: Science / 6th grade
School Year: 2015-2016
Directions:
This form is a tool to assist teachers in setting a goal that results in measurable learner progress. NOTE:
When applicable, learner achievement/progress should be the focus of the goal. Enter information
electronically into the cells (the boxes will expand to fit the text).

Initial Goal Submission (due by 10/05/2015 to the evaluator)

I. Setting (Describe the population
and special learning
circumstances)

II. Content/Subject/Field Area

(The area/topic addressed based
on learner achievement, data
analysis, or observational data)

III. Baseline Data (What is shown

by the current data?)

William Monroe Middle School is located in rural

Stanardsville in Greene County, Virginia. There are just
under 280 6th graders enrolled for the 2015-2016 school
year. There are 76 students spread throughout 4 blocks with
me, with 13 out of the 76 receiving additional services
during a collaboration class with Mrs. Lotterhos. Each
student comes into the classroom having completed a 5th
grade science cirriculum.
This classroom is addressing the scientific standards of
learning 6.1 through 6.9. There are all levels of
achievement from high-level learners to struggling learners
present in the classroom. One block is a collaboration
classroom, with 13/23 students needing extra
accommodations throughout their learning areas.
Students are expected to be able to correctly determine both
precise and approximate measurements by both using and
reading different instruments. They are also expected to
know what each instrument is, what its purpose is, and
when it is an appropriate time to use each instrument.
The topic that will be addressed throughout the year is
Science SOL 6.1b -

For the pre-test, students scored correct answers 45% of the

time for the 7 questions dealing with standard 6.1b.
Question 1 was answered correctly 39.7% of the time.
Question 2 was answered correctly 38.4% of the time.
Question 3 was answered correctly 50.7% of the time.
Question 4 was answered correctly 21.9% of the time.
Question 5 was answered correctly 67.1% of the time.
Question 6 was answered correctly 56.2% of the time.
Question 7 was answered correctly 41.1% of the time.

IV. Goal Statement (Describe what

you want learners/program to

accomplish)

By spring of 2016, 80% of my students will show

measurable growth in the SOL standard of 6.1b by scoring
at least a 30% increase in the number of questions answered
correctly on the pre- and post-test assessments.
Furthermore, 90% of my students will be able to identify
instruments used during experiments.

V. Means for Attaining Goal (Strategies used to accomplish the goal)

Labs/Experiments

Target Date

Students will participate in the

following:
Exit slips
Kahoot (to practice
measurements and metrics)
Graduated
cylinder/Volume of an
irregular solid lab
Measurement stations
Triple beam balance
gizmo
Measurement &
Instrument instructional video
Measurement &
metric jeopardy game
Measurement &
metric notes created together
Assorted practice for
homework
Foldables
Every time a lab or experiment is
conducted, students will revisit this
topic to decide what instrument we will
probably be using and why that is the
correct choice. They will then have
practice using those instruments
throughout the year.
Students will be given a quiz where
they will have to identify instruments,
read instruments, and correct a fake
test answer problem explaining their
reasoning as to why the test answer was
incorrect.
Students who need additional help will
receive it both after school, before
school during homeroom, and during
Dragon Time whenever it is asked for
or as needed.

First 9 weeks

Every time a lab

or experiment is
done they will
revisit this skill.

End of the 5th

week

Based off of
quiz scores, exit
slips, and
requested time.

answers 67.6% of the time for the 7 questions
dealing with standard 6.1b. It was an overall
increase of 22.6%.
Question 1 was answered correctly 78.4% of the
time. This was an increase of 38.7%.
Question 2 was answered correctly 58.1% of the
time. This was an increase of 19.7%.
Question 3 was answered correctly 87.8% of the
time. This was an increase of 37.1%.
Question 4 was answered correctly 73.0% of the
time. This was an increase of 51.1%.
Question 5 was answered correctly 56.8% of the
time. This was a decrease of 10.3%.
Question 6 was answered correctly 62.2% of the
time. This was an increase of 6.0%.
