One significant learning experience I had was how to collaborate with

interdisciplinary health care team. Nursing is not just about the
focusing on the clients current needs, its about ensuring all necessary

members of the healthcare team can do everything for their patient,

from ensuring adequate diet, to anticipating future health needs. One
example of this would be ensuring a mental health patient has support
systems in place before being discharged from the hospital. The use of
home care, primary physician and support groups can drastically
increase the outcome for patients. Had this patient been sent home
without these supports they could relapse, and any progress to manage
or treat their mental health disease could be ruined.
I learnt that collaborating with the interdisciplinary health care team
can have a significantly better outcome for patients, not just in mental
health but in all aspects of nursing care. Many different studies have
shown the effectiveness of working with interdisciplinary health care

teams, they have shown improved patient outcomes, shortened length

of stay, and decreased workload for physicians with the emergence of
nurse practitioners, and a cost savings. This relates to the competency
as you will experience this every day in your nursing practice, working
in a hospital setting you will work with physicians, dieticians, social
work and many other interdisciplinary team members. The use of
SBAR will also ensure effective communication to different members
of the interdisciplinary team.

Good; I understand the importance of working with interdisciplinary

team members. It is important to work with them advocate for your
clients health, improve patient outcomes and assist them in not only
their health needs but emotional needs as well. As an example, taking
physician orders, assisting patient with recommendations from social
work, dietician and assisting in setting up homecare would all be
examples of working with the interdisciplinary team members.
I will use this everyday in my nursing, as I work with patients with
varying conditions the interdisciplinary healthcare team will change. It
may include dieticians, social work, physicians in order to advocate for

my patients health, and ensure they get the support they need. Using
the SBAR technique will also assist me in effective communication
with the interdisciplinary team, in order relay vital information to other
members of the team. I will get the hands on knowledge as I work with
different clients everyday to alter my care for patients with assistance
from the interdisciplinary team.
