AGREEMENT FOR BORROW AND RETURN OFFICE EQUIPMENT
I,, am borrowing the following equipment (the
Equipment) from Ushra Kota Sdn Bhd. : 1 unit Lenovo Laptop Model 80NJ with Notebook Bag
The rental period for my equipment is from .to. . I understand
that :
1. It is my responsibility to pick up the Equipment, and to return it to the same location at
the end of my rental period;
2. I agree to return the Equipment to UshraKota in the same condition that it was in on the
day I rented it;
3. I understand that there is no charge for the rental of the Equipment. However, in the event
that the Equipment is lost or destroyed during my rental period, I agree to replace the
Equipment at my own expenses or; compensate the same cost of the Equipment
purchases RM;
4. If the Equipment is damage during my rental period, I agree to have the Equipment fixed
or replaced.
