Narrative of Offense Committed
On or about the above stated date and time, the Defendant did

operate a motor vehicle by one or more of the following means:

a. While under the influence of an alcoholic beverage or drugs or a
combination of such substances;
b. While having an alcohol concentration of .08 or more as measured in the person's breath, blood or
urine; and having been convicted of/received a deferred judgment for such an offense twice before; the said defendant having been
convicted of/received a deferred judgment for the crime of Operating While Intoxicated in CarrollCounty, Iowa Cause No. 02141 OWCR009096
on December 16, 2007; and the said defendant having been convicted of/received a deferred judgment for the crime of Operating While
Intoxicated in Calhoun County, Iowa Cause No. 02131 OWCR503941 on January 13, 2009.

STATE OF IOWA,

I, the undersigned, being duly sworn, state that all facts contained in this Complaint and Affidavit, known by me or told to me by other reliable persons form the basis for my
belief that the defendant committed this crime
State all facts and persons relied upon supporting elements of alleged crime

On June 10, 2016 at approximately 7:15PM the Sac County Communication Center received a call stating there was red Jeep Grand Cherokee
traveling west on Highway 20 all over the road. I was traveling east on Highway 20 when I met this vehicle and observed a child with there hands
hanging out the wind. I then turned around on the vehicle and observed the vehicle cross the center line on 2 different occasions and hit the fog line
once. I then activated my emergency lights and conducted a traffic stop. The vehicle took a few seconds to pull over. I then approached the vehicle on
the driver's side and observed a female, later identified as MB, sitting in the driver's seat and a small child sitting in the backseat. I explained to MB the
reason I stopped her was she crossed the center line. While speaking to MB I observed she had blood shot watery eyes and had a slurred speech. I
then asked MB to step out of the vehicle to have her perform field sobriety maneuvers. MB advised she would. I then demonstrated and explained the
maneuvers. MB failed all three tests. MB declined the PBT. Based on my observations MB was placed under arrest for OWI and transported to the Sac
County Jail.
