Presenting Problem
Kim Johnson is a forty-two-year-old woman who was referred by the local church due to
her substance abuse. Mrs. Johnson currently lives in a single-family home with her husband,
Robert. Mrs. Johnson has had an ongoing substance abuse problem and has had multiple
accidents at their home. Mrs. Johnson is a stay at home wife, and Mr. Johnson has come home on
multiple occasions to Mrs. Johnson passed out from drinking. Mrs. Johnson has said that the
drinking is causing problems at home and has agreed there is a problem.
Mrs. Johnson made it apparent that there are many stress factors that have been linked to
the binge drinking, Mrs. and Mr. Johnson have had on-going financial issues that have caused a
great deal of stress in their daily lives. Mr. Johnson has tried to get Mrs. Johnson to look for a job
but Mrs. Johnson feels she is unable to apply/interview for jobs due to her current state. Mrs.
Johnson is afraid that alcohol may affect any interview/job she may get and is looking for help.
Mrs. Johnson mentioned that her husband has threatened their marriage if she does not seek help.
Mrs. Johnson also stated that her husband and she had tried to conceive a child for years before
finding out she is infertile and is unable to ever conceive children. Mrs. Johnson claim that
started the abuse.
Family of Origin History
Mrs. and Mr. Johnson have been married for twenty years. Mrs. Johnson has a close
relationship with both of her parents and sees them on a weekly basis. Mrs. Johnsons mother has
mentioned that her daughter has a substance abuse problem and has caused tension.
Mrs. and Mr. Johnson had tried to conceive a child for many years before finding out it
was impossible for Mrs. Johnson to ever conceive and carry a child. This devastating news

caused the client to become depressed. The news also started causing fights between Mr. and
Mrs. Johnson.
Mrs. Johnson has no siblings and has no in-laws. Mr. Johnsons parents both died very
early on in his life and never had any other children. Mrs. Johnson felt like she had no one to talk
to when she was going through all of her child-baring issues and now feels she should have seen
a therapist.
Mrs. Johnsons parents are both very support of Mr. and Mrs. Johnson and have helped
them financially for years.
Developmental History
Mrs. Johnson was born prematurely at thirty weeks and weighed only three pounds.
Premature birth caused Mrs. Johnson stayed in the NICU for ten weeks, due to medical issues.
Mrs. Johnson was late hitting milestones all throughout toddlerhood and did have to see a speech
specialist in kindergarten. Mrs. Johnson was given everything necessary to live a healthy and
normal life. Mrs. Johnson was very late in started her menstrual cycle, starting at the age of 15
years old. Every menstrual cycle since her first has been very irregular.
Health History
Mrs. Johnson premature birth caused her to develop Bronchopulmonary dysplasia (BPD).
This disease affects the lungs and causes asthma symptoms throughout life. Mrs. Johnson is also
diagnosed as infertile.
Relationship/Sexual History
Mrs. Johnson has had a social life. She had many friends growing up and was very social.
Mrs. Johnson has only had one relationship and sexual partner, which is her current husband.
Current Relationship
Mr. and Mrs. Johnson have been married for thirty years. They have had many arguments
in the past due to her drinking. Mr. Johnson is very worried about his wifes drinking and
supports her getting her the help she needs.

Mrs. Johnson started drinking around the age of thirty after finding out she is unable to
conceive a child. The drinking continued for many years and became worse after running into
financial problems. Mrs. Johnson says she drinks at least one bottle of wine a day and will often
open a second. There have been reports of consuming over two bottles of wine in one day.
Mental Health History
Mrs. Johnson has been reported to have depression. The depression developed after
receiving the news that she is unable conceives a child. Mrs. Johnson reports that she is still
depressed.
Military History
Mrs. Johnson has no military history.
Financial History
Mrs. Johnson has had no financial issues growing up. Her family was wealthy and lived
in well-kept homes. Recently Mr. and Mrs. Johnson have had financial issues due to only one
income in the home.
Legal History
Mrs. Johnson has no legal history.
Immigration Issues
Mrs. Johnson has no immigration issues, as she was born in the United States.
