Presenting problem
Brett Favre is a 42-year-old male that is referred by his employer due to his chronic
prescription drug abuse. Mr. Favre Lives with his wife or twenty years and their 15-year-old son.
Mr. Favre has been prescribed opiates for pain relief for over twenty years. Over the twenty
years, he has developed an addiction and recently it has caused problems in his family life as
well as his professional life. Mr. Favre began taking pain killers for his chronic shoulder pain as
a result of a high school football injury. Mr. Favre has been taking double the amount of pills he
is prescribed. He has been cited on numerous occasions by family and co-workers as too
intoxicated to function at the level we need him to. Mr. Favre understands he has a problem and
is voluntarily seeking help. Mr. Favre began taking pain killers for his shoulder pain but now
admits he uses them he feels stressed.
Mr. Favre points to several things that have been increasing the stress in his life, and as a
result he uses. He feels work is the most stress inducer in his life, the constant pressure to
perform has taken its toll on him. Problems in his professional life have impacted his family life
as well. Mr. Favre is having relationship problems with his wife, and also having trouble
connecting with his son. After a long day at work, Brett uses to relax, he then goes home and is
unable to function as the husband and father his wife wants him too.
Family of Origin History

Mr. Favres mother was 33, and his father was 35 at the time of his birth. At the time, the
couple had been married for 12 years. His mother is now 75 and father 77, and still happily
married. The two are in great health for their ages and take minimal medications to treat high
blood pressure. They both are high school graduates; it was after high school his father started
his first business roofing houses.
Mr. Favre has one brother named John, who is 41, single, and works as a carpenter. John
is very active himself and remains in great health with no complications. The pregnancies of
both children were planned. The two remain in contact and spend time together at least once a
week.
The atmosphere was that of a typical white, middle-class American family. Mr. Favre
remembers having fun and experiencing adventure as a child. His family all gathered at his house
for holidays and birthday celebrations. The family was not exposed to a great deal of tragedy
throughout Mr. Favres life. When his grandfather died, it was the first time they all had to face
death together. It pulled the family even closer once they lost a loved one.
Developmental History
Mr. Favres mother gave birth to him after an unremarkable 40-week gestational period.
At time of birth, he weighed 9lbs, 3ounces. The birth was a vaginal delivery. He was breast fed
until one year of age. He first walked around fifteen months, began talking around two, and was
potty trained by age three and a half.
Health History
Mr. Favre is in overall good health. He was born a healthy baby with no complications.
He suffered several ear infections as an adolescent but nothing more serious than that. Mr. Favre

broke his arm at eight years old which required surgical repair with a plate and screws. He later
tore his rotator cuff during a high school football game. Mr. Favre remained healthy and active
from childhood up to his current age. No pertinent family illnesses are present.
Relationship/Sexual History
Mr. Favre was a popular child with many friends. He was the star of his high school
football team and liked around the neighborhood. He is currently friends with many of the same
people that he was during his childhood. He began dating his freshman year of high school at age
14. He is a heterosexual. His first sexual experience was at age fourteen with his first girlfriend
whom he dated for the following two years. He reports that he enjoyed sex then and still does
now, with no reported problems. The couple used protection until his wife had a hysterectomy
following the birth of their son. Mr. Favre has no sexually transmitted diseases and has had three
sexual partners to date.
Family of Creation/ Current Relationship History
Mr. Favre had two previous committed relationships before marrying his wife of twentyyears. He was twenty two when he married his wife who is currently forty years old. The two
have a 15-year-old son named Wyatt. Mr. Favre and his wife report relationship issues stemming
from his substance abuse problem. His relationship with his son seems to suffer as well; they do
not have a close father-son relationship. Mr. Favre holds a masters degree from an accredited
university. His wife holds a high school diploma and his son is currently in high school. His wife
experienced a healthy gestation period, and there were no complications at birth. The couple
planned the conception of their child and looked forward to it for a few years before going
forward. There has never been any history of abuse or violence in the home.

Substance Use/Abuse History


Mr. Favre was prescribed pain medication twenty-two years ago for shoulder pain when
he was twenty-years old. His usage has increased over time, and he openly admits to abusing the
medication for over a decade now. Mr. Favre continues to use and is seeking help to regain
control. His previous substance abuse counseling did not make any changes in life; he did not
take it seriously. Mr. Favre uses no other controlled substances.
Mental Health History
Mr. Favre has seen counselors on and off throughout his life when he is feeling
depressed. He went to a local mental health clinic that he was referred to by a friend to receive
treatment. He felt better after speaking with someone about how he was feeling. No medications
were ever prescribed for the periods of depression.
Military History
Mr. Favre has no military history.
Financial History
Mr. Favre comes from a Caucasian middle-class family. His father was a successful
business owner, and his mother was a housewife. His father was able to earn enough to support
the family, take vacations, and own a lake home for the family to go to on the weekends.
Mr. Favres personal finances are in good health. He can support his family while his
wife stays at home to focus on raising their child. He has a high paying, stable career.
Legal History

Mr. Favre has no prior arrests or convictions. He received a citation two years ago for
being intoxicated in public and ordered to complete ten hours of substance abuse counseling.
Immigration Issues
Mr. Favre is a legal United States citizen. He was born and raised in the United States.
Treatment Plan
Goal One: Mr. Favre will stop abusing pain killers.
Objective A: Mr. Favre will learn techniques to cope with addiction.
1. Mr. Favre will attend Narcotics Anonymous meeting twice a week for eight
weeks.
2. Mr. Favre will purchase the book How to Quit Drugs for Good: A Complete
Self-Help Guide and read before the beginning of the third week of
counseling.
Objective B: Mr. Favre explore alternatives for pain relief.
1. Mr. Favre will see prescribing physician before beginning the second week of
counseling to discuss alternative medication and physical therapy.
2. Mr. Favre will work with a physical therapist for eight weeks, if approved by
primary care physician.
Goal Two: Mr. Favre will improve his relationship with his family.
Objective A: Mr. Favre will improve communication with his family.
1. Mr. Favre will make five attempts per day to openly communicate with his
wife and son for the remainder of the treatment period.
2. Mr. Favre will attend therapy for eight weeks, taking the techniques learn and
applying them to his personal relationships.
Objective B: Mr. Favre will learn how to be more affectionate with his family.
1. Mr. Favre will purchase and read the book Five Love Languages before the
beginning of the fourth week of counseling.
2. Mr. Favre will attend one family counseling session per week for eight weeks.

1. Description of the Problem: Mr. Favre is suffering from an addiction to pain killers. Mr.
Favre has increasingly abused pain killers for many years and it is taking its toll on him
as well as his relationships. Mr. Favre struggles in his marriage and being a father. He has
shutdown communication with his wife and son. Their relationships are in poor health.
2. Primary Goals: Help Mr. Favre become and stay sober. Also, improve Mr. Favres
relationship with his wife and son.
