I, Mariam bt Ali, (NRIC No: 770412-10-1234) a Malaysian citizen of full age

of No. 45, Jalan Platinum 7/7A, Seksyen 7, 40000 Shah Alam, Selangor Darul
Ehsan do hereby affirm and say as follows:

1. I am the said Judgment Debtor.

2. Unless otherwise stated, the facts deposed herein are true and within
my knowledge.

3. I crave leave of this Honourable court to refer to the Creditor’s Petition

dated 1st day of March 2010 (hereinafter referred to as the said Petition)
which is annexed as ‘CP-1’.

4. The defect of the said Petition is caused by the absence of the

authorization in the said Petition.

5. Next, there is a failure of deponent to attest affidavit to the said

Petition.

6. Based on my above statements, I verily believe there are issues to be

tried.

7. As such, I humbly apply to this Honourable court that the Creditor’s

Petition dated 1st day of March 2010 which was served to me on the 13th
 day of March 2010 by Bank Muslim (Co. No: 123456), the Judgment
Creditor, to be opposed.
