Pneumonia Vaccine is recommended for:

 Everyone 65 years of age and older
 Individuals with a chronic illness, asthma and cigarette smokers

PLEASE NOTE:
 Be sure to wear short sleeves/loose fitting clothing
 There is no charge for the vaccines, however, insurance information is necessary
 MEDICARE, AETNA & CIGNA insurance cards must be presented
 Pregnant women must provide a note from their obstetrician
 Minors must be accompanied by a parent or legal guardian
