Checklist of Application Documents

1. University Application Forms:
Application forms could be Paper-Based or to be filled Online. Paper-based forms will
be provided by Career Mosaic. Assistance for filling out online forms will be provided at
Career Mosaic premises.

2. 10th and 12th Standard Mark Sheet & Transcripts:

• True Copies attested by School Principal in white envelope sealed by the School
authorities.

3. College Transcripts:
• True Copies attested by College Principal / Register / Controller of Exam, in white
envelope sealed by the College/University Authority.

4. College Yearly/Semester wise Mark Sheet:

•True Copies attested by the College Principal / Registrar / Controller of Exams, in
white envelope sealed by the College / University Authority.

5. Official OR Provisional Degree Certificate:

•True Copies attested by the College Principal / Registrar / Controller of Exams, in white
Envelope sealed by the College / University Authority.

6. Three Letters of Recommendation:

• For Undergrad applicants, 2 School Recommendation’s are ok.
• For Grade Applicants, 2 College Recommendation & 1 work Recommendation.
NOTE: Recommendation Letters must be on the School/College Letterhead.

7. Statement of Purpose/Essay & Resume:

To be prepared as per guidelines and sample.

8. Bank Statement:
On the official Bank Letterhead (Nationalized/Private) with original seal & signature of
the Bank Manager.

9. Affidavit of Support:
On Rs.20/-, Rs.50/- or Rs.100/- Stamp paper as per the sample provide.

10. TOEFL/IELTS/GRE/GMAT/SAT Scores:

Photocopy of official/unofficial score sheet OR Print of Exam Date
Confirmation.

11. Certificates of Extra Curricular Activities/Achievements:

Note: Only Photocopies, not to be attested.

12. Syllabus/ Course Work / Curriculum of the Degree / Diploma earned:

Note: Stamp of School/ College on the first page of photocopy syllabus.

13. Photocopy of Passport:

First & last page of the passport. Not to be attested.
NOTE: All documents listed above will be needed in multiple copies, as per number of
universities the student is applying to. Please arrange for these documents as soon as
possible and get them checked by your Counselor. Delay in preparing the documents can
influence your admission process.
