1. I am submitting the attached photo which I personally took as my entry in the

Thunderbird Resorts-Express Your Art Photo Contest.

2. I will abide by the Mechanics, official Rules, and the decision of the Judges,
which I will accept as the final decision.

3. My failure to comply with the rules of the contest will result in my disqualification.

4. By joining the Contest, I hereby release and hold the Organizers and their
employees, officers, directors, agents, representatives, its advertising, promotion,
and fulfillment agencies, and legal advisors, harmless from any and all losses,
damages, rights, claims and actions of any kind in connection with the prizes and
giveaways; or resulting from acceptance, possession, or use of any prize,
including without limitation, personal injury, death, and property damage, and
claims based on publicity rights, defamation, or invasion of privacy.

5. In the event that my entry is declared as one of the winners, I hereby allow the
Organizers and its designees the use of my name and/or photograph, for
advertising and publicity purposes.

6. I will be responsible for any and all other incidental expenses associated with the
collection of the prize in the event that my entry is declared as one of the
winners.
