THE SPIRIT OF FHP AWARD

The Spirit of FHP Award will be presented to a full-time, post-secondary student in the
Fitness and Health Promotion program at Fanshawe College, on the basis of the following
selection criteria:
 successful completion of the first year of FHP and be progressing satisfactorily through the
program
 good academic standing with no failures on transcript
 student must exemplify the spirit of the FHP program as evidenced by his/her determination,
positive attitude, interaction with peers, community and/or faculty members and overall
commitment and contribution to the program
Application forms are to be returned to the Awards and Scholarships office, Fanshawe College, Room
K1003, by Friday July 29, 2011. The Spirit of FHP Award will be presented at the annual Awards
Ceremony held in October.
Incomplete applications will not be accepted. Please complete application in ink.
