Isolated ATSI male with suicidal ideation and intent with low mood presenting in
context of substance abuse and relationship stressors.

- Using the Mental Status Examination clearly explain the signs and symptoms the
client

- Identify stressors and/or factors that might have contributed to the client’s problems. 
Relationship breakdown , death of relative, substance abuse, isolation, job stress,
hopelessness, nihilistic beliefs, child of the stolen generation

- Identify the client’s strengths and capacities. 

Has job, has relationship, supportive mother, community nurse, willing to engage.

- Identify at least 2 complete nursing diagnoses.

- 1) Poor self-esteem re to loss and grief.
2) Potential for harm re to suicidal ideation and hopelessness.
3) Deteriorating mood and ADL's re to increased sub abuse and social withdrawal

Substance abuse can be used to develop Dx of deteriorating mood and disinhibiting

suicidality. There are hundreds but focus on the risks and on problem solving.

- Describe the risks involved in caring for this person. 

High prev of death in custody when admitted to institutions, may need constant obs,
limited local health providers, may need transfer to bigger place of safety, larger unit.
Vol vs involuntary adm. Cultural issuse as per above.

- Identify expected outcomes for this client.

That they are singular, realistic and achievable; observable, measurable,
behavioural, time designated

- Identify the types of treatments considered appropriate for your client. 

Relationship counselling, home care if family willing and able risk assessed as low,
private psychologist, Drug and Alcohol Service referral, admission to ward, review by
Reg, Consultant, close obs, Medication, etc.

- Discuss the process of developing a therapeutic relationship with this client. Pay
particular attention to the difficulties you might encounter. As per the above.

- Describe and explain the types of problems that the client might experience in the
future.

 Increasing Isolation and withdrawal, increasing substance abuse, deterioating mood,

Self Harm, Death
- Identify the legal and ethical issues of concern in this case. Hospital policies re to
admission and Obs of ATSI clients, Mental health Act , Nursing Act ,Duty of Care,
Litigation, Patient rights, Least restrictive polices, confidentiality
