ATTENDANCE / PUNCTUALITY

An employees absence not only affects patient care, but the working conditions of fellow employees and company expectations. Therefore, a high premium is placed on attendance and punctuality. Regular attendance during all scheduled hours at work, reporting to work on time and continuing to work to the end of the work period is expected of every employee. A good attendance record is an indication of your interest in your work and this record will be reviewed when evaluations are given. Each employee is required to be at the assigned work area ready to begin job activities at the beginning of the scheduled shift. Attendance Guidelines: Appear for work no sooner than 5 minutes prior to the start of the shift and no later than the start of the shift. With Time Sheet or Electronic Time cards, you may clock in upto 5 minutes prior to your work schedule but you must not work until your scheduled time. You will not be paid for any time prior to the start of your schedule. Being at your work station ready for work by the start of the shift is required. Remain at your work station unless the needs of the job require being elsewhere. Remain at work during your entire shift, unless excused by a supervisor. Do not leaving work until the scheduled end of your shift, unless excused/approved by a supervisor. Leave promptly at the end of your shift, unless you have been given advance permission/approval from your supervisor to work past that point; and Call in and personally notifying your supervisor or another member of management if you are going to be either absent or tardy, unless a verifiable emergency makes it impossible for you to do so.

************************************************************************************************************* Tardiness/early leave -defined as reporting to work late at the beginning of the work shift, leaving early, returning late from lunch, leaving work early at the end of the work day, and/or changing your schedule all without approval. Tardiness/early leave will result in unauthorized leave without pay. Approved time will also result in time without pay. * The number of minutes and / or hours an employee misses or is tardy for work. from lunch, leave early etc. will be subtracted from the time sheet and not calculated towards PTO time. Excessive tardiness/early leave (excused or unexcused) will result in appropriate disciplinary action including but not limited to termination. ************************************************************************************************************ Absence - defined as not showing up for work or leaving early from work due to sickness or personal emergency etc. Employees who are going to be absent due to sickness, personal emergency or are going to be late must call and talk with their supervisor as soon as possible (prior to 1 hour before shift starts, but no later than the start of their shift.) If the supervisor is unavailable, employees may leave a message for the supervisor with a phone number where they can be reached. Family or friends may NOT call in an absence for an employee unless the employee is physically unable to make the call. The supervisor will then return their call. If the employee does not receive a return call within 2 hours, the employee is expected to call the Human Resources Department. If you are absent without notice for two days in a row or two separate days throughout the year, you agree and will

consider it submitting a voluntary termination and the Company will process your work separation as a voluntary resignation on your part. Notification of absence or being tardy does not exempt an employee from review for disciplinary action. Notification of absences does not necessarily mean the absence will be excused, PTO assigned or paid. Absenteeism, tardiness, early leave will be documented and will be grounds for disciplinary action, including but not limited to termination. To avoid misunderstanding, an employee should ask Human Resources about specific attendance and call-in procedures in their department. We may require supporting documentation when attendance issues arise. Not supporting the proper documentation requested can lead to disciplinary action, including but not limited to termination. *******************************************************************************************************

Tardy/Absence/Leave early beyond PTO(PTO guidelines applicable) - January December. It will be conducted on a Points system. Point Earn for each situation: 2 Points Tardy/Leave early (with notice) early 1-60 minutes 3 Points Tardy/Leave early (with notice) early more than 60min. (Tardy over an hour is considered an absence.) 3 Points Absence - (with notice) Unpaid 5 Points Tardy, Leave early (without notice) Notice 5 Points Absence (without notice) (2 consecutive days = voluntary termination) Disciplinary Action Taken: (accumulative points) 5 Points Earned Verbal Warning 7 Points Earned Written Warning 10 Points Earned Termination Levels may excel or skip base on accumulated points. Absences outside of approved PTO will be affected by the attendance policy.
