Generic & Brand Name Mode of Action Mechanism of Action Indication & Contraindication Indication: Treatment of primary or secondary gout, hyperuricemia resulting from chemotherapy for malignancies, recurrent calcium oxalate renal calculi. Recurrent tophaceous deposits or uric acid stones. Contraindication: Severe renal impairment, and children except those with hyperuricemia secondary to malignancy, idiopathic hemochromatosis, acute gouty attack. Hypersensitivity. Lactation. Uricosurics
Inhibits xanthine oxidase, an enzyme involved in the synthesis of uric acid without disrupting the biosynthesis of essential purine. Results in decreased uric acid level.
Assess patients history, gout may be secondary to diseases such as acute or chronic leukemia, Adverse Reactions: CNS: headache, drowsiness, polycythemia vera, multiple myeloma or peripheral neuropathy, psoriasis neuritis Assess for pain including Dermatologic: rashes location, characteristics, GI: nausea, vomiting, onset/duration, frequency, diarrhea, abdominal pain, quality, intensity or gastritis, hepatomegaly. severity of pain, precipitating factors Hyperbilirubinemia, Monitor uric acid levels cholestatic jaundice every 2 weeks GU: exacerbation of gout Monitor renal function and renal calculi, renal Monitor CBC and hepatic failure function at the start of Hematologic: anemia, therapy and periodically leucopenia, agranulocytosis, thereafter Monitor nutritional status thrombocytopenia Be alert for adverse reaction and drug interaction Tell patient to increase fluid intake to 2L/day and to maintain a neutral of alkaline urine Tell patient to stop drug and report if skin rash, stomatitis, malaise and fever occur
