Sir, It is humble submitted here that Sri Prabhu Barik who was working as a Handling Labour, previously in FSD/F.C.I., C.T.O. under DM, Mayapur, New Delhi, is died / unknown fraudulently Sri Dhusana Barik entered the name of Sri Prabhu Barik in F.C.I. office Register. Now Sri Dhusasana Barik, Son of Sri Late Radhashayam Barik, At/Post: Mahadev Bast, G.P.- Patta Sindurapur, Block/P.S. Kakatapur, Dist: Puri, Orissa had been working in the name of Prabhu Barik as a Mondal in FSD/FCI, Ranital under D.M. Balasore, Orissa Region having some malafied and ill intention. He had declared himself as Sri Prabhu Barik instead of Sru Dhusasan Barik. We got information from reliable sources that Sri Prabhu Barik is working in the address of Dusasan Barik. It is further humbly prayed that your honour may issue show cause notice the direction to the Collector Puri Orissa to identify the person Prabhu Barik as the same Dhusasana Barik verifying with proper identity by Voter Identity Card before and after appointment in FCI on the date 01.05.1984, whose date of Birth 14-01-1961 and further issue direction to the Police Station Kakatapur to investigated the matter and take necessary action regarding falsification of public document and cheating to the public official his appointment in FCI record.
