Entry Form for Teens Grades 6 thru 12
Please use this form for your final entry. You may practice on another sheet of paper before you color your final design.

Rules and Guidelines

Your bookmark design should be original. Any designs using characters that are copyrighted, such as Disney cartoons or the original cover of the book will not be considered when judging the winners. Please use markers, crayons, or pens when drawing your designs. Do not use pencils, stickers, or any other decorations. We need to consider what will show up best when others print your winning bookmark.

Any cover artwork depicting inappropriate material for display will be eliminated. Entries will be accepted at any library location until September 14, 2012. Prizes will be awarded in two categories: Middle School (Grades 6 to 8) and High School (Grades 9 to 12) 1st Prize 2nd Prize 3rd Prize $50 cash $25 cash $10 cash
