NSO Birth Certificate

Birth Certificate (BC) in Security Paper (SECPA) issued by the National Statistics Office (NSO) or Certified True Copy (CTC) of BC issued by the Local Civil Registrar duly authenticated by NSO depending on the NSO signatory. For Born before 1950 If his birth certificate has been destroyed, damaged or is not available due to other causes, a baptismal certificate for those who are members of a Christian religious organization or similar or equivalent certificate issued by a non-Christian religious group, attesting to the applicant's having been admitted to such religious group or sect at an early age and where it is indicated that the applicant is a Filipino citizen, which should be accompanied by a joint affidavit by two persons who have personal knowledge of the applicant and of such age as to credibly state that the applicant's date and place of birth, citizenship, and names of parents. Filipinos who do not subscribe to any religion and whose parents for any reason failed to have the said applicant baptized shall be exempted from the requirement to present a baptismal certificate as prescribed in the preceding paragraph. In lieu thereof, the applicant shall execute an affidavit to that effect duly corroborated by the affidavit of at least (2) persons of good reputation who personally know such fact. If his birth certificate has been destroyed damaged or is not available due to other causes, he shall be required to reconstruct the same by filing a delayed registration of birth with the Local Civil Registrar. In case of applicants born outside Metro Manila, the delayed registration of birth may be filed with the National Statistics Office (NSO).

Other Supporting Documents which may be required by the Secretary or any authorized representative or consular officer, for the purpose of verifying the identity, citizenship and personal circumstances of the applicant such as: Marriage Contract Land Title Driver's License GSIS/SSS Records School Form 137 or Transcript of Records with dry seal Other documents that show full name and birth details of applicant and/or citizenship Voter's Registration Record from COMELEC Intramuros Baptismal Certificate with dry seal Seaman's Book Income Tax Return (Old)

Original Valid ID
Original ID i.e. Senior Citizen's ID, Voter's ID, digitized government-issued IDs like SSS, PRC, BIR, Driver's License, original school ID (for students only) AND original supporting documents indicating full name, date and place of birth and citizenship.

If your Birth Certificate was Late Registered:


If registered on or before 2000: submit 1 original supporting document indicating full name, date and place of birth, and citizenship prior to the date of late registration. If registered on or after 2001: submit 2 original supporting documents indicating full name, date and place of birth, and citizenship prior to the date of late registration.

Certified True Copy of Birth Certificate


Certified True Copy (CTC) of BC issued by the Local Civil Registrar with dry seal if BC SECPA is unreadable
