5. The List of Student Educators should be given to the RVP for Academics with their duly accomplished hour of participation. This is for the issuance of Certificates of Recognition as Student Educators. Student educators should complete a minimum of nine (9) hours for them to be given certificates. 6. For monitoring purposes, Monthly Reports should be submitted at the end of each month to the RVP Academics email address. The Monthly report shall contain the following: Narrative Report Documentations List of Student Educators (with their duly accomplished hours) Attendance sheet of tutees a. July 29, 2012- Submission of first monthly report b. August 26, 2012- Submission of second monthly report c. September 30, 2012- Submission of third monthly report 7. To give due recognition and appreciation to the Local Chapters which heed to this program with their whole and earnest attention, the Office of the Regional Vice President for Academics decided to give additional two points to the total points of the LC which launched their SSGP on July 4, 2012 and submitted their SSGP plan and the Accomplishment report on the Launching on July 15, 2012. 2. For the Monthly report, additional one point shall be given to the LCs who will submit their report on time. V. EVALUATION 1. Evaluation sheets for the SSGP activities of the Local Chapter shall be presented and given to the Local Chapter Presidents. All participants of the SSGP Programs of the Local Chapter shall be given an evaluation sheet. 2. The evaluation sheet for the SSGP activities of the Local Chapter shall only be given to the students on the last scheduled date of the LCs SSGP Program that they participated in. 3. Evaluation sheets for the Top SSGP educator shall be given to the tutees in all sessions that the applicant has participated into. VI. QUALIFICATION, REQUIREMENTS AND CRITERIA FOR JUDGING Top SSGP The search for the Top SSGP-Local Chapter recognizes the Local Chapter that has performed best in providing its members the most effective Student Study Group Program and established an academic friendly environment for its members to reach their full-academic potentials.

Top Student Educator The search for the Top Student Educator recognizes the educator who has exemplary performed his duties in providing his co-JPIAns an avenue to learn and who has offered his time and invaluable efforts to explore and harness academic potentials of his co-members. Each LC may send a maximum of three nominees. o Qualifications Must be a bona fide member of the federation 2012-2013. No non-passing grades in all subjects taken. GWA of 80% or above or its equivalents. No dropped subjects. The candidate must not be involved in any criminal case brought upon by any competent course of jurisdiction. Requirements in Hard Copy Duly signed endorsement letters from the incumbent JPIA Advisers Certified Copy of grades One page hand-written essay discussing Why should I be the Top Educator? Documentation in relation to the activities of SSGP. Video of actual SSGP session of educator (with a minimum of 30 minutes) Criteria for Judging Essay Evaluation Board of Judges Total Judges Evaluation Academic Performance and Achievements Educator Qualities Total 10% 20% 70% 100% 30% 70% 100%
