RM 10212.200 Changing Numident Data for Reasons other than Name Change
A. How does SSA change Numident Data
SSA does not correct or change information that is on a prior Numident record other than documented keying errors. Based on a new application and supporting evidence, a new record showing the new data is appended to the prior record(s) on the Numident. B. What is required to Correct or Change Numident Data


1. Request evidence Each applicant requesting a change or correction of information on the prior Numident record must submit evidence: of identity establishing that he or she is the person on the record to whom SSA assigned the SSN, and supporting the change. If the application is an Enumeration System application, do not presume there was a keying error; you must obtain a new application with proper evidence 2. Obtain Documentation Use this table to determine what documentation supports the requested change and what actions are necessary.

Sex-Change Operation: Applicant must submit a letter from his or her surgeon or the attending physician verifying that the sex change surgery was completed. All documents must clearly identify the NH. NOTE: Use the Numident sex data for identification purposes only. Do not use the sex data shown on the Numident to determine whether a valid marital relationship exists in a claims situation. Consult appropriate State law in order to make a determination on whether a valid marital relationship exists. Keying Error: The prior SSN application must be reviewed to correct a keying error, do not presume there was a keying error. If the prior application can not be reviewed, obtain a new application with proper evidence. Applicant must submit evidence showing the correct sex and the evidence must have been established before the CYD date on the Numident with the incorrect entry.

Date of birth or Place of Birth

U.S. born, a birth certificate, or Foreign born, an acceptable document such as a birth certificate or an acceptable proof of age document listed in RM 10210.265, provided the document also lists place of birth. (non-relevant data edited from handout to conserve paper)

3. Process the Request Process the requested change but suppress the issuance of a replacement SSN card when the correction will not affect any data on the face of the SSN card and the NH is still in possession of the SSN card showing the correct data.
