The purpose of the DPAS application is to provide an electronic means of conducting the DPAS II evaluation process. Each teacher has a Forms page which currently includes the Pre-Observation and Professional Responsibilities Forms that are required for teachers to complete as part of the evaluation process. In addition, the application also includes the forms that evaluators complete as part of the process including the Formative Feedback Form and the Summative Evaluation Form. The application also generates an email that will inform you as to when meetings have been scheduled and provide you with a link to appropriate forms, etc. Please see the end of this QuickTips for samples of the emails you will receive.

Please noteAll draft forms will be highlighted in blue. All forms requiring review will be highlighted in yellow.
Pre-Observation Form To Add a Form, click on Add New Form from the Teacher Forms screen Click in the text box under each section and enter your response.
All forms are equipped with an AutoSave function that will automatically save your draft every 3 minutes. Please make sure you still save your document when finished.
Tip: To check for spelling errors, click on the Spell Check icon
If you are not done, you can Save as Draft and continuing working on it at a later time. When complete, click on Submit Form to Evaluator. Note: Once a form is submitted to evaluator it cannot be changed unless pushed back by the evaluator. Once you have submitted the form to the evaluator, you will get a confirmation screen similar to the one shown here. You will be able to upload documents from this screen to be attached to your forms. Note: Once you click Submit Form to Evaluator, an email will be sent to the evaluator for his/her review. The evaluator can either Push Back the Form to you for further edits or Accept the form. You will receive an email for either action. 3
Print/Upload Documents To print the form or upload documents click on the Print/ Upload link from the View My Info screen
To upload a document, you may name the document in the comments field, then search for the document by clicking on browse. Select the document you wish to upload and click Upload Document
Note: Once a document has been uploaded there will be a paperclip indicating that there is an attachment. You may click on the paperclip link to view the document.
Professional Responsibilities Form To Add a Form, click on Add New Form from the Teacher Forms screen Click in the text box under each section and enter your response.

Confirming Formative Evaluations Teachers will get an email once an evaluator submits an Formative Feedback Form. To view the form, click on Forms to Review or View My Info.
Click view next to the formative evaluation form to view the form.
Once you have reviewed the formative evaluation, click on confirm at the bottom of the page
A confirmation screen will appear once confirmed. Click ok if you are sure you want to confirm. Click cancel to return to the formative feedback form. An email will be sent to the evaluator informing them that the form has been reviewed.
Once the form is reviewed and confirmed you can create a PDF or upload documents.

To view the actual Pre-Test Assessment, click on Pre-Test link The selected assessment details will appear.

Important: Teachers should view ALL documents in order to ensure they have all the necessary tools to administer the assessments
Once logged into DSC Web Application (www.dataservice.org), click DPAS & Walkthrough
Select the Subject, Grade and Assessment then click on the check boxes for the courses to which you will administer the assessment. Click Save.
Once you have saved your choices, you can Edit your selection by clicking on the Edit link. Click the Add New button to add additional Assessment and Course selections.

Definition
Teacher of Record gives grades in math and/or reading for at least 10 students in DCAS tested grades 3-10. Teacher of Record gives grades for at least 10 students in any grade level or subject other than DCAS reading and/or math grades 3-10. Any educator who does not meet the criteria of Group 1 or Group 2 will defer to Group 3. Task Group 1 Group 2 Group 3
Electronically Verify Rosters Select Courses that will be tested with DCAS Select External and/or Internal Measure B assessments and Classes Select Growth Goal Measures
Roster Verification (Group 1 and Group 2 Educators) Click on the View Current Students link to view the students in each class. If all class lists are correct, click the check box to mark that the roster has been verified. Important: If a roster for a class is incorrect, notify the school office in order for the correction to be made in eSchoolPlus.
Measure A Selection (Group 1 Educators) Click either the Math or Reading column next to the classes which are DCAS tested classes.
Measure B Selection (Groups 1 and Group 2 Educators) For Internal Measure B Assessments, select the Subject, Grade and Assessment, then select the class to which you will administer the selected assessment. Finally, click the Add Internal Measure B button. For External Measure B Assessments, select the External Measure then the classes to which you will administer the assessment, then click the Add External Measure B button. Once the form is completed, either Save as Draft or Submit Form to Evaluator. Selected Measure B assessments will be listed below the selection boxes.
Measure C Selection (Group 2 and Group 3 Educators) Click on the drop down to Choose Subject. A Subject specific list of DOE approved Growth Goals will appear. Review each goal then click on the check box for the goals that you have selected then click the Add Measure C button. The selected Measure C Growth Goals will appear below the selection boxes. Once the form is completed, either Save as Draft or Submit Form to Evaluator. Evaluators can either accept the form, or push it back to the teacher or specialist for changes. IMPORTANT : Percentages provided with Growth Goal Measures are recommendations provided by DOE.

If a skip question has all zeros in the bubbles, online data entry in PerformancePLUS is required. These questions are 6 points or higher and do not fit the default bubble sheet option of 1-5. All data entry must be completed after scan forms have been scanned and scores have been uploaded to PerformancePLUS by DSC. The Data Service Center will notify all schools once the scanning is complete and data entry can take place. Open-Ended Questions With NO Zeros
At least five (5) days before administering assessments, review scan forms and identify missing or incorrect forms. Contact the DSC Helpdesk if there are any issues.
Use a No. 2 pencil only. Encourage students to thoroughly & carefully erase changes in answers. Monitor students while testing in order to ensure no one marks on the barcodes.
If a skip question has no zeros in the bubbles, and the scoring rubric includes zero as a valid score, do NOT bubble any value. Blank=0; 1=1; 2=2; 3=3; 4=4; 5=5
If missing scan forms for a class and/or individual students, contact DSC at helpdesk@dataservice.org, (302)504-7222 or (800) 866-5927.
Do not enter open ended scores in PerformancePLUS before scan form data is loaded. Do not print scan forms at the school. DSC will print all scan forms. Do not staple, rubber band, or paper clip scan forms together. Do not three hole punch scan forms. Do not affix post-it notes to scan form. Do not use pen or red pencil. Do not send blank scan forms back to DSC for processing. Do not spill coffee or other liquids on scan forms.
Only send original scan forms back to DSC for processing. Scan forms should all be facing the same direction. Returns scan forms as soon as possible.
