1. PURPOSE : 1.1
To keep Classified Information pertaining to patients identity, illness that may affect the integrity of a person.

2. DEFINITION :
2.1 It is a process that involves keeping information related to a persons pertinent data en trusted to an individual or institution. 3. POLICY: 3.1 Under any circumstances any information about patient may not be disclosed without proper authorization as covered by Hospital Laws, Policies and Procedures. 3.2 Patient information access through the computer is considered the same as the patients medical record and cannot bring into open, without the permission from proper authority. 3.3 Patient information maybe used for teaching process only when authorized by the institution and in compliance with policy and procedure. 3.4 There shall be no posting in public of patients information, e.g. Name, Diagnosis and so forth. 3.5 Ensures continuous security and confidentiality in accordance with policies and procedures.

4.1 Releasing of any information concerning patients is done through proper channel by authorized personnel : 4.1.1 Treating Doctor. 4.1.2 Social Worker. 4.1.3 Nursing Staff ( Related to Nursing aspects.)

4.2 Name and diagnosis of the patient are excluded in the bedside tag and door tag. 4.3 Patients data in chart tag is attached at the inside of the chart cover.
