1. PURPOSE:

1.1 To ensure safety of the newborn. 1.2 Correct identification of newborn.


1.3 Protection of exchange of newborn.

2. DEFINITION :
2.1 The process by which a worker verifies the fact data are the same as the one described on patient recognition on Physicians order.

3. POLICY: 3.1 ID band serves as the official newborn identification and are before initiating any procedure on that newborn and on discharge from the unit 3.2 The ID band should correspond to the newborn data on the chart. 3.3 The ID band should be placed on newborn ankle or wrist and a matching ID band should be placed on the mothers wrist before leaving the birth area. 3.4 ID band should be water proof and the data must be written clearly and must match on what is written on mothers ID band.

4. PROCEDURES: 4.1 After cleaning the newborn, apply the ID band both to the baby and the mother.

4.2 Placed ID band on newborn wrist and ankle must have the following entries : 4.2.1 Medical Record Number. 4.2.2 First name : Baby boy / baby girl / mothers name. 4.2.3 Last name.

4.3 Securely apply the ID band to the newborn wrist and ankle, this pose challenges be cause the band is very small and the newborn skin is fragile. 4.4 Make sure that the data printed on the ID band is clear so reading is accurate and legible. 4.5 Mother and newborn are identified from delivery through a special set of ID band. This band must remain until discharge. 4.6 The nurses should check the ID band for matching information everytime, every shift during endorsement, upon admission and discharge, before giving the medication, and before any procedure. 4.7 Take the newborn footprints and the mothers thumbmark before leaving the birth area. 4.8 The ID band are checked by all nurses to validate that the correct newborn is brought to the right mother if they are separated for any period of time.

5. FORMS AND EQUIPMENTS : 5.1 Water proof ID bracelet / Band. 5.2 Birth Certificate.
