CHRONIC DISEASE PREVENTION COORDINATOR - FULL TIME POSITION Mamaweswen, THE NORTH SHORE TRIBAL COUNCIL invites applications for the
position of Chronic Disease Prevention Coordinator to work out of the NSTC Head Office in Cutler, Ontario and throughout member First Nation and Aboriginal partner sites.

Purpose:
To provide a leadership role through coordination and support of the Health Units Chronic Management Disease program by linking, implementation monitoring and administering the following programs; Diabetes, Alcohols Spectrum Disorders, Fetal Alcohol Spectrum Education Partnership Project and Health Eating and Active Living programs.

Primary Duties and Responsibilities:


The development and submission of reporting requirement to the various funders as well as follow up on reporting deadlines for community submissions as outlined in the memorandum of agreements. Effectively monitor the status of chronic disease within the member First Nations, utilizing findings in the maintenance of health and level functioning; ensuring ongoing evaluation of health status; developing reports for Supervisor and other health care professionals as required. Assisting the development of local and regional policies and procedures related to chronic disease management. Coordinate Diabetic services, implementing and assisting the Diabetes Management Team in designing quality programs while meeting the needs of diabetic clients. Provides administration assistance to the Health Director or designate in recruitment and hiring of chronic disease personnel as established through the organizational policies and procedures. . Develops and practices effective supervision, support and employee coaching strategies. Address change to the program when challenges arise involving travel requirements, fiscal restraints, staff retention and recruitment and funding agreement changes. . Monitors assigned financial statements of the chronic disease programs. Provides financial support to the finance committee for health by participating budget discussions pertaining to programs. Completes the reporting requirements for the funders including NE LHINS.

Knowledge of program development, implementation, monitoring and evaluation. Experience working with Regional or Tribal services, and health planning committees. Knowledge of data collection analysis, policy development and program planning. Demonstrated sensitivity to and knowledge of First Nation Culture values and traditions. Knowledge of Anishnawbe Health and Traditional Health Practices. Excellent oral and written communication skills. Must have skills in working in a multi-disciplinary team environment and understand the concepts and behaviours of being a team player. Networking and working in collaboration with external health care agencies and organizations. Ability to advocate.

Qualifications: Registered Practical Nurse Diploma or certificate (mandatory) Registered in good standing with the College of Nurses of Ontario (CNO) and/or the Registered Practical Nurses Association (RPNAO). Three to five years in nursing practice along with some expertise in the promotion of healthy lifestyles, smoke free strategies and diabetes prevention. Knowledge of measuring tools utilized in the Quality Management Program. Knowledge of community resources connected to the provision of chronic disease prevention related services. Strong organizational and analytical skills with an attentiveness to detail. Knowledge with understanding of Aboriginal and First Nations culture and of traditional medicine. Work effectively, both independently and as part of an inter-disciplinary team. Flexibility and willingness to adapt to change. Strong computer skills, i.e. Microsoft Office.. Ability to work flexible hours (evening and weekend work will be required). Possess a valid Ontario Drivers License. $ 40,000 $45,000 No later than 4:00pm on December 19th 2012 (Late applications will not be considered)
