AFFIDAVIT OF SINGLENESS

I, EMY ANANIAS YANESA, single, of legal age, Filipino citizen, and a resident of 1211 Vicente Basit St.,Daet, Camarines Norte, after having been duly sworn to in accordance with law, hereby depose and say: That I am of legal age, having been born on June 11, 1979, in San Lorenzo (formerly Imelda), Camarines Norte; That sometime on date, I had an elicit relationship with a man with whom I begot a son born on July 29, 2001 at Doa Marta Maternity Hospital in Pasay City, Metro Manila, and baptized by the name of Ronwel Yanesa Hernandez; That in the Birth Certificate of my aforementioned child, it was erroneously recorded that I am married, when in fact I have not been married to any man before; That I am executing this affidavit to attest to the truth that I am single, and have never been married before, in compliance with the requirement for my intended marriage and for any other legal purpose this may serve.
