Introduction
Welcome to e-LAB Solutions, the most valuable provider of online Proficiency Testing (PT) and Lab Accreditation Program (LAP) information available. Software Requirements
For details regarding the appropriate browser versions and supplemental software to use the CAP Web site, go to About CAP > Technical Environment.
Laboratory staff:
All laboratory staff is responsible for: Creating their own Web accounts on www.cap.org Requesting access to laboratory data
3. This will bring you to the New User Registration page. Enter your name and then choose a unique user ID and password. All fields marked with the red asterisk must be filled before you can continue to the next page. Click Continue.
4. Enter your email address in the space provided for Preferred e-mail address. Select 3 different questions from our drop-down list of questions and provide answers. The answers you provide will be used to verify your identity if you require assistance with your account in the future. 5. Provide either a Business or Home address to continue.
You will see the message below when you have successfully created an account. You should be notified by e-mail within one business day that your web account has been activated.
Note: If you do not receive this email and you know that a new user has requested access, please have your IT department verify that acknowledgement@cap.org is on your safe senders list.
Once you have an active web account, you can log in to the CAP website with your User ID and Password. 1. Go to www.cap.org and log in with your user ID and password.
Click Hint if you need to display the hint you provided when you set up your account
2. The CAP Web site will show a welcome message and a list of personalized
options once youre logged in. Here you can change your password or update your profile when necessary.
4. At the entry screen, enter your organizations seven-digit CAP number, and then click Submit.
6. Your request will be sent to the site administrator(s) to grant or deny access. 7. Once you have been granted or denied access, you will receive an e-mail message detailing your access privileges (functions you have been authorized to use by your site administrator).
Selecting a Laboratory (if you have access to more than one lab)
If you have access to more than one laboratorys data, you must first choose which laboratorys data you want to view. If you have access to only one laboratory, it is selected for you automatically. 1. Log in (see Creating individual Web accounts and logging in). 2. Click on Proficiency Testing/ Quality Management under the e-LAB Solutions list. 3. Click on Select or Change Laboratory. All of the laboratories that have granted you access are listed. 4. Select the radio button next to the appropriate organization and click Select Lab. Then click Return to e-Lab Solutions after the line becomes highlighted. The page will update and display information about your selected organization at the top of the page.
3. Enter the 7-digit Laboratory ID (CAP number) and PIN provided on your opt in letter and click Submit. Note: Each lab director has been sent a PIN. If you do not have your PIN available, contact the CAP at 800-323-4040 option 1.

4. Read the Terms and Conditions. To continue you must click Accept.
5. The confirmation screen below will open to show that your laboratory has been activated for online Web access. You have been designated as a site administrator for the laboratory, and are now responsible for managing access privileges for staff in your laboratory. You must now specify which CAP programs you want to enable for online access. Please make your selections and click Submit. You can update these selections at any time by clicking on Administration Options under the e-LAB Solutions heading on the left side of the page and then Select Programs.

Note: The CAP recommends that each institution have at least two site administrators. Use Manage Permissions to designate another site administrator (see Managing Permissions). Managing Permissions (for site administrators only)
Site administrators are responsible for maintaining access privileges for laboratory staff. This can be done when staff first request access and as needed in the future.
New requests
When laboratory staff request access to your laboratorys data, you will receive an email notification. Note: If you do not receive this email and you know that a new user has
requested access, please have your IT department verify that acknowledgement@cap.org is on your safe senders list.
1. Go to www.cap.org and log in. Select your Laboratory (if applicable). 2. Go to Administration Options under e-LAB Solutions. Click on New Requests.

3. To grant access for an individual, click on Specify Employee Access to the right of the persons name and email address. Note: Selecting Specify Consultant Access will limit the individuals access to view-only for proficiency testing results. If the individual needs access to enter results, claim CE/ CME, or view CAP accreditation materials please choose Specify Employee Access.
4. At the next screen, click on the appropriate checkboxes to select the privileges you want this person to have. Click Update Privileges at the bottom. Note: After you grant access privileges for an individual, an e-mail message will automatically be sent to the requester letting him or her know what access privileges they have been granted.

3. Click on the link for the users name. This will bring you to the Maintain Access Privileges screen. To update access for an individual, click on the appropriate checkboxes to select or deselect privileges and click Update Privileges. To revoke access, click Revoke All Privileges.

4. After you grant or deny access privileges for an individual, an e-mail message will automatically be sent to him or her about the access privileges you changed for them.
Getting there
1. Log in, under the e-LAB Solutions header, click Proficiency Testing/Quality Management. Select Result Forms under the My Laboratory header. Depending on the number of items in the list for your laboratory, you may see a window open letting you know that the list is being created.

2. Choose a kit from the list or use the filter options to change what kits are listed. The default setting is to list kits based on their upcoming due dates for kits that have been shipped in the last 60 days.
By default, the list is sorted by kits due within the next 60 days.
3. Click View under Details to access that kit. 4. The details screen provides more information about the selected kit:

Product name, mailing code, kit number, and due date are displayed in this area.
Entering data
Each individual page is listed with kit status and receipt information. The Data column displays individual privileges (the ability to enter/view data) and the status of the kit. (For example, if a page has a status of Not Received, the option under the Data column will be Enter Data for a person who has that access. This assumes that the due date for that kit has not passed yet.) Related links include help, return to the kit list, download a blank result form, view/print your saved data, view the kit transaction history (which provides additional details about the kit), and kit instructions.
To enter data, look for the Enter Data link in the Data column of the Result Form Details page. Click on that link to open up the result form online.
Viewing data
To view data, click on the View or the View/Edit link in the Data column. Links in the Data column will reflect what actions you can perform depending on your access privileges and the status of the kit page (whether the page is waiting approval or has been approved). The links will open a PDF version of the result form.

To enter or verify instruments or methods, place your cursor in the Instrument or Method Code field and a drop-down box will open to provide you with an alphabetical list of valid instruments and codes. Only codes from this list may be used. If your instrument or method is not listed, enter 010 in the corresponding boxes and proceed after saving your data to the last page of the Result Form. Under Use of Other, provide the name of the instrument or method you use.

The bottom of each page features buttons that enable you to save data and navigate to other pages of the result form.
Approving Data
A button will display with Approve Pending Pages when there are kit pages to be approved and submitted if you have been assigned the authority to approve data. Site Administrator or other designated individual with the authority to approve pending pages will see the screen pictured below:

In order to approve the pending pages, you must click on the Approve Pending Pages button in the orange box in the screen above. Until this step has been completed, the results have not been submitted to CAP and will remain in pending approval status in the result form detail page. Notes about data entry and submission: Data can be edited any time prior to evaluation (by personnel with the appropriate privileges). Approved and submitted data will be analyzed by the CAP once approximately 80% of the result forms have been received. Your site administrator(s) will receive an e-mail to alert them that a product is ready to evaluate and your data hasnt been received. This notification is emailed to the site administrator(s) 5 days prior to the printed due date on your result form.
If you need to know who entered data and when, click on View kit transaction history on the kit summary page. It will show the user id of the person who entered the data.

Select a mailing name from the list or use the filter options to change what kits are listed. 4. Click View details to access information for a specific kit.
By default, kits evaluated within the last 60 days are displayed.

5. Once a kit is selected the details screen provides a selection of reports available for that particular kit. The evaluation report mailed to your organization is available in PDF format, the Interactive Evaluation allows for individual analyte viewing. With the Available Reports filter, you can select an evaluation report type. 6. To select another kit click on Return to kit list.

You will only have access to the above links if you are the Site Administrator or you have been given access.

This report will let you see a compiled version of scorecard data currently on file for your laboratory. You can modify your report by using the Options/Filter tools. 1. Report Type allows viewing of CMS regulated analytes or all analytes including non-regulated analytes. 2. Number of Events defaults to 3 events, but allows viewing of 1 or more events (up to 6 events - - 2 years of data). 3. Grade allows viewing of all, acceptable, unacceptable or PT performance <100% (LAP) grades. 4. Mailing allows viewing of enrolled mailings by discipline (ex. Mycology). 5. and 6. Subspecialty and Analyte - allows viewing by specific subspecialty and analytes within that Subspecialty (must be used in conjunction with the Mailing field). Once the Options/Filters have been chosen, click View Filtered Report to view the report or click Reset Form to choose new Options/Filters.

1. Subspecialty/Analyte lists the individual analytes under a given subspecialty (the first rows under the subspecialty reflect the overall score of all reported analytes that contribute to the subspecialty). 2. Year and Event lists the year and event (mailing) that you are looking at. 3. Eval Date the date the analyte was evaluated. 4. Kit Number kit identifier in which a given analyte appears (this is an interactive link, clicking on it takes you to the evaluation). 5. Survey designates Survey code and mailing event. 6. Score/%/Performance lists the overall contributing challenges for a given analyte, score percentage and performance (satisfactory or unsatisfactory). 7. Test Name name of the analyte (also an interactive link, clicking on it takes you to the evaluation page in which the analyte appears). By closing the Filtered Report, you will be taken back to the Options/Filter screen.

This report shows which regulated analytes the lab is or is not reporting to CMS. It is a static image. The (More info) link instructs on how to use/make changes to the report.

The first part of the report will list regulated analytes the lab is enrolled in, but does not perform. 1. Subspecialty/Analyte lists the individual analytes under a given subspecialty. 2. CAP No. lists the laboratorys CAP identifying number. 3. Kit/PFG (Product Fulfillment Group) lists the kit number and PFG the analyte is being reported from. 4. Event/Report lists the event (mailing) and whether it is being reported or not as indicated by a Y (Yes) or N (No). 5. Indicate Changes the lab would indicate if they would like to change the reporting status (if a change is being made to Y, the lab must indicate what PFG it is to be reported from). The second part of the report lists which regulated analytes the lab is enrolled in and performs for CMS regulations and looks like this:
1. Subspecialty/Analyte lists the individual analytes under a given subspecialty. 2. CAP No. lists the laboratorys CAP identifying number. 3. Kit/PFG (Product Fulfillment Group) lists the kit number and PFG the analyte is being reported from. 4. Event/Report lists the event (mailing) and whether it is being reported or not as indicated by a Y (Yes) or N (No). 5. Other PFG/Indicate Changes indicates other PFGs that the analyte is contained in and a line to write the PFG reporting change.

CAP #/ LAB Name lists the laboratorys CAP # and name. City, State lists the laboratorys city and state. Ship Date lists the date the product ships out. Mailing/Product lists the specific mailing and product. Qty lists the quantity of kits that were ordered. During order renewal, you will able to see two calendars the current year and the upcoming year. After January 1st , only the current years calendar will appear.

If your laboratory is missing required PT, a Y will appear in the Missing PT Enrollment column. You can select the link to the PT Enrollment Guide to determine PT product options.

Additional Reports
Additional reports are available. These can be viewed, saved and printed by selecting the link on the upper-right hand corner of the report.
While viewing the Laboratory Activity Menu Report, the user will be able to print it or save it as a .pdf or an Excel spreadsheet. You will also be able to click links that open the laboratorys Checklist Selection Report, the Master Activity Menu report (specific to the program in which the lab is enrolled), a dynamic Missing PT Enrollment Report (more accurate and potentially different from the version that runs monthly and is mailed), PT Enrollment Guide and, the Test Menu Change Form.

The Master Activity Menu can be viewed in either an Adobe .pdf file or a Microsoft Excel file. Selecting the Excel format will allow you to conveniently sort and manipulate the data as needed.

The following steps will show you how to select your section/department (e.g. General Chemistry), checklist module (e.g. Chemistry and Toxicology), checklist edition (e.g. Self-Inspection), checklist type (e.g. Custom), and checklist format (e.g. Excel/CSV) and generate a checklist for viewing or saving. You may generate as many checklists as you desire, one at a time.

3. Select a Section/Depte.g. Bacteriology, Blood Bank, etc. All the sections/departments for your laboratory are listed. Your selection in this box controls which modules are displayed in the Checklist Module box. If you would like to view a list of all modules, including those that dont currently pertain to the sections/departments within your laboratory, select List All Modules.

If you selected List All Modules in the Section/Dept dropdown menu, then all checklist modules, even those that dont pertain to the sections/departments within your laboratory, will be available for viewing/downloading.

Onsite InspectionThe checklist requirements used by your laboratory and an inspector during a scheduled onsite inspection. FAQs Can I download more than one checklist? You may download multiple checklists, for various modules, in the various editions, types, and formats, but you must do so one at a time. What version of Adobe Acrobat Reader do I need to download PDF checklists? Adobe Acrobat 6.0 or higher is required. What version of Microsoft Word do I need to download Word checklists? Word 2003 or higher is required. What is the Word/XML (Windows Only) checklist format? Word/XML format is supported by Microsoft Office 2003 and 2007 for Windows. When a checklist is viewed or saved, the file will have a .doc file extension. At this time Mac users are only able to use the PDF and Excel/CSV formats. What is the Excel/CSV checklist format? I would like to view all the checklist modules, not just those pertaining to my laboratory. How do I do this? In the Section/Dept box, select List All Modules.
